Wiederherstellung der weiblichen Brust nach Mastektomie

Zusammenfassung

Grundlagen: Die Rekonstruktionsmöglichkeiten der weiblichen Brust haben sich in den letzten Jahren verändert. Ein historischer überblick sowie unsere eigene Erfahrung, vor allem mit dem LD-Lappen, werden dargestellt.

Methodik: In 9 Jahren haben wir bei 143 Patientinnen 150 Brustrekonstruktionen, vor allem sekundär, durchgeführt. Dies erfolgte durch Einsetzen einer Prothese, eines Expanders, durch autologes gewebe allein oder in Verbindung mit einer Prothese.

Ergebnisse: Bei 37 Patientinnen wurde die Brust lediglich mit einer Prothese rekonstruiert, davon bei 4 Frauen nach vorheriger Expanderimplantation. Bei 5 Patientinnen wurde nur eine erweiterte LD-Lappenplastik durchgeführt. Bei 101 Patientinnen wurde zusätzlich zur Lappenplastik eine Prothese implantiert. In 5% traten postoperative Komplikationen auf (Nachblutung, Epitheliolyse, Lappenspitzennekrose).

Als Spätkomplikationen beobachteten wir in 7% Prothesendislokationen und in 10% eine korrekturbedürftige Kapselfibrose. Bei einer Umfrage bezüglich des kosmetischen Ergebnisses mit einer Rücklaufquote von 89% zeigten sich 75% der Patientinnen sehr und 22% relativ zufrieden.

Schlußfolgerungen: In unserem Krankengut stellt die LD-Lappenplastik mit und ohne Prothesenimplantation noch immer die zahlenmäßig am häufigsten durchgeführte Rekonstruktionsmethode dar, da wir zeigen konnten, daß das Funktionsdefizit an der Entnahmestelle des Muskels und die Komplikationsrate sehr gering sind.

Summary

Background: The possibilities of reconstruction of the female breast have changed over the past years. A history of techniques as well as our own experience, especially with the latissimus dorsi flap, are presented.

Methods: In 9 years we have performed 150 mainly secondary breast reconstructions in 143 patients with prosthesis only, with an expander, with autologous tissue only or combined with a prothesis.

Results: 37 patients underwent reconstruction of the breast by implantation of a prosthesis, 4 of them primarily had a tissue expander. In 5 patients an extended latissimus dorsi flap alone was used. In 101 patients a prosthesis was placed under the myocutaneous flap. Postoperative complications occured in 5% (haemorrhage, epitheliolysis, partial flap necrosis). As late complications we observed 7% dislocations of prothesis and 10% fibrosis of capsula requiring corrective surgery. In an inquiry 75% of the patients said to be very and 22% moderately satisfied with the cosmetic result. The reponserate was 89%.

Conclusions: In our series the latissimus dorsi flap without of with prothesis remains the most frequently used method for breast reconstruction since we could show that the functional impairment of the donor site is minimal and the complication rate is very low.
