Zusammenfassung
Grundlagen: Die modifiziert radikale Mastektomie mit Sofortrekonstruktion hat sich unter dem Aspekt einer Verringerung des psychischen Traumas und einer stärkeren Beachtung der Lebensqualität zu einem etablierten Verfahren entwickelt. Durch die Sofortrekonstruktion ergeben sich keine Nachteile im Krankheitsverlauf, weshalb die Indikation auch auf fortgeschrittene Tumorstadien ausgeweitet wurde.
Methodik: Als Rekonstruktionsmethode kommt die einfache Silikonkissenimplantation, die Expandermethode, der Latissimuslappen und der Transverse Rektus Abdominis Muskel (TRAM) — Lappen in Frage.
Ergebnisse: Mit den beiden erstgenannten Methoden ist nicht immer eine natürliche Brustform zu erzielen. Bei geplanter postoperativer Bestrahlung sind diese Methoden unserer Meinung nach kontraindiziert, da durch die Induration des Weichteilmantels eine Expanderfüllung unmöglich werden kann bzw. das kosmetische Ergebnis leidet. Mit den Lappenplastiken lassen sich auch größere Brüste mit ausgezeichneten Resultaten nachbilden. Beim TRAM-Lappen kann auf die Verwendung eines Silikonimplantates verzichtet werden. Durch die identische Konsistenz mit dem Mammagewebe können Form und Ptose ideal wiederhergestellt werden. Eine Bestrahlung der Lappenplastik ist möglich, beeinflußt jedoch das kosmetische Spätergebnis, weshalb im Falle einer Radiatio einer Spätrekonstruktion im Anschluß an die Bestrahlungsserie der Vorzug zu geben ist. Vom Standpunkt der mammographischen Rezidivkontrolle ist der Eigengewebsrekonstruktion der Vorzug zu geben, weil kein störendes Implantat die Beurteilung beeinträchtigt.
Schlußfolgerungen: Durch Verbesserung der Techniken der Sofortrekonstruktion kann das psychische Trauma reduziert und das Körperbild erhalten werden. Ob eine Expandertechnik oder eine aufwendige Lappenplastik zur Anwendung kommt, wird vor allem von der Infrastruktur der behandelnden Abteilung abhängen.
Summary
Background: Modified radical mastectomy with immediate breast reconstruction has become an integral part of breast cancer therapy. This procedure is able to reduce psychological trauma and may provide increased life quality after surgery without mutilation. Concerning the further course of the cancer disease, no disadvantages related to immediate breast reconstruction could be shown by several studies.
Methods: A number of reconstructive procedures is available and should be individually selected. There is a broad scope from simple and straight forwald techniques like silicone implants and skin expanders, up to myocutaneous flaps such as latissimus dorsi flap or transversus rectus abdominis muscle flap (TRAM).
Results: By using silicone implants or skin expanders it is not always possible to achieve a natural breast shape. If postoperative irradiation is scheduled, these methodes are contraindicated in our opinion, because the induration of the soft tissue makes the expander — filling impossible or reduces the aesthetic result. Even big breasts may be reconstructed excellently when applying myocutaneous flaps. The TRAM Flap does not need a silicone implant for shaping. Because of the identical consistence with the breast tissue, shape and ptosis are ideally reconstructable. Irradiation of the flap is possible, but influences the cosmetic outcome, therefore in cases of planned post-operativ radiation we prefer a delayed reconstruction. The TRAM Flap gives the best condition for mammographic recurrence control, because, there is no implant which may reduce the ability for judgement.
Conclusions: By improving the techniques of immediate breast reconstruction it is possible to preserve the integrity of the body. The decision if an expander technique or a complex flap will be used, will be mainly influenced by the infrastructure of the department.
