Zusammenfassung
Wir berichten über eine zum Unfallzeitpunkt 47jährige Patientin, die 1981 an einer veralteten Tibiakopffraktur mit ausgedehnter Defektzone (41-B3.1, lateraler Impressionsspaltbruch) in unserer Klinik operiert wurde. Zur Rekonstruktion der völlig zerstörten Gelenkfläche diente dabei ein freies Patellatransplantat. Nach 16 Jahren ist die Patientin bei einem sehr guten funktionellen Ergebnis klinisch nahezu beschwerdefrei. Dieser Langzeitverlauf unterstreicht eindrucksvoll, daß die Patellatransplantation den Wiederaufbau von ausgedehnten Defekten im Tibiakopfplateau, die mit keiner anderen Methode zu rekonstruieren sind, ermöglicht und insbesondere beim jüngeren Patienten eine erfolgversprechende Alternative zum prothetischen Gelenkersatz darstellt.
Abstract
A case of surgical reconstruction of the lateral tibial plateau after impression fracture (Type 41-B3.1) of an 47-year-old female is presented. Joint reconstruction was performed by using a free patellar graft. The 16-year follow-up shows a very good functional outcome without subjective complaints. This case underlines that performing patellar autografting for surgical reconstruction of severe injury of the tibial plateau seems to be a promising alternative especially considerating young patients in spite of avoiding total knee replacement.
