Die traumatische Zwerchfellruptur

Zusammenfassung

Anhand von 21 eigenen Fällen wird über Entstehungsmechanismus, Pathophysiologie, Symptomatik, Diagnostik, Differentialdiagnose, Therapie und die Ergebnisse bei traumatischen Zwerchfellrupturen eingegangen.

Die Diagnose des Krankheitsbildes wird als Hauptproblem in den Vordergrund gestellt. Zur Vermeidung von Fehldiagnosen ist wichtig, an die Möglichkeit der Zwerchfellruptur zu denken, vor allem bei polytraumatisierten Patienten und bei Frakturen im Beckenbereich. Die routinemäßige Inspektion und Palpation des Zwerchfelles bei nach Unfällen notwendig gewordenen Thorakotomien und Laparotomien und die regelmäßige Anfertigung von Lungenübersichtsaufnahmen vor und nach der Entlassung von Polytraumatisierten tragen zum raschen Erkennen der Diagnose bei.

Summary

Using 21 of our own cases as examples, Pathophysiology, symptomatis, diagnostic, differential diagnosis, therapy and the results of traumatic diaphragmatic ruptures were investigated using formation mechanisms.

The diagnosis of the illness is placed in the foreground as the main problem. To avoid a wrong diagnosis, it is important to think about the possible occurrence of a diaphragmatic rupture, above all, in polytraumatic patients and in fractures of the pelvic area. The routine inspection and palpitation of the diaphragm thro Thorakotomien and Laparotomien, which have become necessary after accidents, and the regular production of lung surveys before and after the discharge of polytraumatic patients, contribute to a speedy rewgnition of the diagnosis.

Etude à partir de 21 cas des mécanismes d’apparition, de la physiologie pathologique, de la séméiologie, du diagnostic, du diagnostic différentiel, de la thérapeutique et des résultats lors de ruptures traumatiques du diaphragme.

La difficulté majeure réside dans la pose du diagnostic. Pour éviter une erreur diagnostique, il est important de penser à la possibilité d’une rupture du diaphragme surtout chez les polytraumatisés et lors de fractures du bassin.

Un diagnostic rapide est supporté par l’inspection et la palpation de routine du diaphragme en cas de traumatismes nécessitant une thoracotomie ou une laparotomie, ainsi que par des radios pulmonnaires systématiques avant et après la sortie des polytraumatisés de l’hôpital.
