Zusammenfassung

Die traumatischen Hämatome der hinteren Schädelgrube sind eine Seltenheit. Dadurch und durch den Ort der Lokalisation werden sie oft übersehen. Anhand von drei Fällen sowie den klinischen Verlaufsformen wird die Dringlichkeit der operativen Therapie gezeigt. Pathologischanatomische Formen sind vor allem beim epiduralen Hämatom unterschiedlich. Durch moderne Untersuchungsmethoden wie Angiographie und Computertomographie ist die Diagnose heute einfacher zu stellen. Prognose und Verlauf hängen vor allem vom Zeitpunkt der Diagnosestellung und von der klinischen Verlaufsform ab. Operativ-technisch ist die Ausräumung bei Beachtung anatomisch-topographischer Verhältnisse kein Problem.

Summary

Traumatic hematomas of the posterior Fossa are extremely rare. For this reason, and due to the erea of localization, such hematomas are often overlooked. The urgency of operative therapy is shown on the basis of 3 cases as well as the clinical course. Pathologicalanatomical forms are varied, especially in the case of epidural hematomas. As the result of modern test methods, such as angiography and computer-tomography, diagnosis is much easier today. Prognosis and course of treatment are dependent on the time at which diagnosis is made and on the clinical course of the patient. The operative therapy presents no problems, if one observes the anatomical-topographical relationships.

Résumé

Les hématomes traumatiques de la fosse postérieure sont extrèmement rares. Ceci ajouté à la localisation fait qu’ils sont souvent méconnus. La nécessité de l’urgence d’une thérapeutique chirurgicale est démontrée à propos de trois cas et des évolutions cliniques. II existe différentes formes anatomo-pathologiques surtout dans les hématomes épiduraux. Le diagnostic est aujourd’hui plus facile à poser, grâce aux nouvelles méthodes d’examen comme l’angiographie et le scanner.

Le pronostic et l’évolution dépendent principalement du moment de la pose du diagnostic et de la forme de l’évolution clinique. La technique opératoire de l’évidement ne pose pas de problème si l’on tient compte des rapports anatomo-topographiques.
