Die Bandplastik der gesprengten tibiofibularen Syndesmose

Zusammenfassung

Die supramalleolären Luxationsbrüche des oberen Sprunggelenkes sind mit einer ausgedehnten Zerreißung der distalen tibio-fibularen Bandverbindung verbunden. Um die Gelenksfunktion wieder zu gewinnen, ist außer der knöchernen Stabilisierung die Rekonstruktion der tibio-fibularen Bandverbindung anzustreben.

Die stark aufgefaserten Bänder erlauben oft keine primäre Naht. In diesen Fällen wird eine plastische Rekonstruktion der tibio-fibularen Syndesmose mit gestielter Sehne des M. peronaeus brevis empfohlen. Die Frühergebnisse zeigten in Anbetracht der Schwere der Verletzung ein gutes Funktionsergebnis.

Summary

Supramalleolar luxation fractures of the upper ankle joint are complicated by expansive tearing of the distal tibio-fibular ligaments. In order to restore joint functioning, the reconstruction of the tibio-fibular ligaments as well as bone stabilization must be accomplished.

The often strongly frayed ligaments do not allow for primary suturing. In such cases, plastic reconstruction of the tibio-fibular syndesmosis through use of the lapped tendon of the peronaeus brevis muscle is recommended. Early results showed a good functional result in view of the seriousness of the injury.

Résumé

Les fractures et dislocations supramalléolaires de l’articulation astragalocalcanéenne supérieure sont liées à un déchirement étendu de la composition péronéotibiale des ligaments située vers l’extrémité. Pour recouvrer la fonction articulatoire, il faut, en plus de la stabilisation osseuse, s’efforcer de reconstruire cette composition péronéotibiale des ligaments.

Les ligaments fortement effilochés ne permettent souvent aucune suture primaire. Dans de telscas, on recommande une reconstruction plastique de la syndesmose péronéotibiale avec tendon pédiculé du M. peronaeus brevis. Les résultats précoces ont montré un bon résultat de fonction par rapport à la gravitè de la lésion.
