Zusammenfassung

An der Chirurgischen Universitätsklinik Mainz wurden von 1965 bis 1976, 66 Patienten wegen eines pHPT operiert. In 25 Fällen (38%) bestand eine ossäre Verlaufsform. Anhand von 7 typischen Beispielen aus dieser Gruppe werden die Gründe der Diagnoseverschleppung, besonders bei im Vordergrund stehenden rheumatoiden Beschwerden, die besonderen diagnostischen Schwierigkeiten bei Vorliegen von Mischformen, die chirurgischen Probleme bei pathologischen Frakturen, sowie die differentialdiagnostischen Kriterien bei tumorösen Neubildungen erörtert und radiologisch dokumentiert. Ferner werden im einzelnen die bei ossären Manifestationen vorliegende Pathophysiologie, sowie die radiologischen und histologischen diagnostischen Möglichkeiten dargestellt.

Summary

From 1965 until 1976 66 patients of the Department of Surgery of the Mainz University were operated on because of pHPT. In 25 cases (38%) bone was affected. 7 typical examples were chosen to dissus and illustrate radiologically late diagnosis especially when rhumatoid signs are predominant, diagnostic difficulties in mixed forms, surgical problems in pathological fractures as well as criteria of differential diagnosis in tumor growth. In addition pathophysiology as well as radiological and histological diagnosis in osseous manifestation is described.

Résumé

Parmi le 66 cas d’hyperparathyroidisme primaire opérés entre 1965 et 1976 dans le service de chirurgie du CHU de Mayence, ón a retrouvé une atteinte osseuse dans 25 des cas (38%). A propos de sept exemples précis tirés de ce groupe, les auteurs discutent le causes d’un diagnostic tardif, surtout en cas de troubles rhumatoides prédominants, les difficultés diegnostiques en prèsence de formes mixtes, les problèmes chirurgicaux de fractures pathologiques, ainsi que les critéres du diagnostic différentiel vis à vis des nèoplasies tumorales, discussion accompagnée d’une documentation radiologique. Les auteurs décrivent également la physiologie pathologique dans les manifestations osseuses ef les possibilités radiologiques et histologiques du diagnostic.
