Zusammenfassung
Für den zivilen konventionellen Katastrophenfall wird ein Dispositiv vorgestellt, durch welches eine große Zahl von Katastrophenopfern kurzfristig und fachgerecht unter Wahrung von Prioritäten behandelt werden kann. Dabei muß auch die weitere Betreuung der bereits in Behandlung befindlichen Patienten sichergestellt sein, Schwerpunkt im Dispositiv ist die organisatorische Umstellung des Normalbetriebes und der gezielte Einsatz der zur Bewältigung der Katastropnensituation herbeigerufenen Ärzte und Pflegekräfte. Hierbei bewährt sich die Alarmierung in 3 Instanzen, wobei eine Abhängigkeit vom Katastrophenausmaß besteht. Besonderer Wert wird auf die Zusammenarbeit mit den Behörden des Katastrophenalarm-Planes der Stadt gelegt, wobei im Mittelpunkt die frühzeitige Information über das Gesamtausmaß der Katastrophe stehen muß. Von der medizinisch-chirurgischen Seite her kommt der sachgemäßen Triage die allergrößte Bedeutung zu. Sie muß vom erfahrensten Arzt vorgenommen werden.
Summary
In the case of a civil conventional catastrophe a scheme is introduced, through which a large number of catastrophe victims can be appropriately treated on a short-tem basis with the assurance that priorities will be observed. In so doing, the further care and control of those patients already undergoing treatment must also be guarenteed. In difficulty in the scheme is the reorganisation of the normal work routine and of the quick response, which one arms for, of the doctors and nursing staff who are called in to cape with the situation of catastrophe. Thus the raising of the alarm proves true in three stages, which are dependent on the extent of the catastrophe. Special importance is placed on the co-operation of these people with those who have the authority to carry out the towns catastrophe alarm-plan, whereby the early information concerning the total extent of the catastrophe must be the cenral point. From the medical-surgical point of view the three appropriate stages are of the greatest importance. They must be carried out by the most experienced doctor.
Résumé
Les auteurs présentent un dispositif pour le cas d’une catastrophe civile conventionnelle, qui permettrait l’hospitalisation immédiate et un traitement optimal d’un grand nombre de victimes de la catastrophe, tout en respectant les prorités. Il faut également assurer les soins des malades déjà hospitalisés.
Le point principal du dispositif est le changement organisatoire de la gestion hospitalière normale et la mise en disposition réfléchie des médecins et soignants auxiliaires appelés pour maitriser la situation de catastrophe. Il est recommandé de donner l’alerte en trois étapes en tenant compte de l’importance de la catastrophe.
On souligne l’importance de la collaboration avec l’administration municipale qui devrait surtout donner des informations le plus tôt possible sur l’importance totale de la catastrophe. Du côté médico-chirurgical, un triage soigneux est de plus grande importance. Il doit être fait par le médecin le plus expérimenté.
