Zusammenfassung
Infolge der Zunahme schwerer Arbeits- und Verkehrsunfälle werden immer häufiger Kombinationsverletzungen der unteren Extremitäten mit Beteiligung der Hüftgelenke beobachtet. Bei den verschiedenen Luxationsformen kann es gleichzeitig zu knöchernen Absprengungen im Bereich der Hüftgelenke kommen. Im Vordergrund stehen die Pfannendachfrakturen. Selten sind die Femurkopfkalottenbrüche mit oder ohne Pfannendachaussprengung. Ihre Differenzierung durch unterschiedliche röntgenologische Einstellungen ermöglicht eine Aussage über Lokalisation, Herkunft und Größe der Fragmente. Dies wurde demonstriert durch experimentell an einem Bekkenpräparat erzeugte Frakturen und einigen charakteristischen klinischen Fällen. Die exakte Diagnose einer Hüftgelenksverletzung ist um so wichtiger, als in den letzten Jahren durch Weiterentwicklung der operativen Versorgung eine weitgehend vollständige Rekonstruktion der Gelenkflächen angestrebt wird.
Eine sofortige Reposition des Hüftgelenkes ist wegen der Gefahr von Gefäß- und Knorpelschädigungen in jedem Fall erforderlich. Das Ausmaß der Verletzungen und das Ergebnis der Primärversorgung sind für die weitere Therapie entscheidend. Zur Wahl stehen konservative und verschiedene operative Möglichkeiten. Unabhängig vom jeweiligen Vorgehen ist eine Extension und langzeitige Entlastung des Hüftgelenks erforderlich.
Die häufig auftretende posttraumatische Coxarthrose nach Hüftgelenksverletzungen kann durch eine exakte Röntgen-Diagnostik und eine auf den Einzelfall ausgerichtete Therapie auf ein Minimum reduziert werden.
Summary
With an increasing number of trafic or work accidents we see lesions of the lower extremity in combination with trauma of the hip more frequently. Several types of hip dislocation are combined with fractures of the hip-joint, especially with fractures of the posterior rim. Fractures of the femoral head on the contrary are a rare condition. A changing direction of the radiographic incidence facilitates a differentiation in localisation, origin and size of the fragments. We were able to prove this by X-ray examination of artificially provoked hip fractures done on anatomic pelvis preparations in our experimental laboratory in comparison with some very characteristic clinical cases. The exact diagnosis of hip lesions now is more important than ever because during the past years an almost complete reconstruction of the joint was made possible by an improved operative technique.
In every cases we do an immediate reposition of the dislocated hip because of possible dammage to vessels and cartilage. The following treatment — either conservative or operative — is dependent on the total amount of lesions and the result of primary treatment. An extension and long time distraction of the hip is necessary in every case independently of any type of treatment.
By exact radiological diagnostic and an individual treatment the up to now higher incidence of posttraumatic coxarthrosis can be lowered striknigly.
Résumé
Avec le nombre croissant des accidents du travail et de la circulation, on observe de plus en plus fréquemment des lésions par combination des extrémités inférieures avec participation des articulations de la hanche. Selon les différentes formes de luxation, il arrive que des éclats osseux se manifestent dans ces régions. Ce sont les fractures du toit du fémur qui sont au premier plan. Les ruptures de la calotte de la tête du fémur avec ou sans éclatement sont rares. La différenciation possible grâce à des attitudes roentgénologiques variées permet de se prononcer sur la localisation, la provenance et la taille des fragments, ce qui a été dêmontrê par des fractures provoquées expérimentalement sur des pelvipréparations et par quelques cas cliniques caractéristiques. Prononcer un diagnostic exact sur une lésion de l’articulation de la hanche est d’autant plus important qu’on vise ces dernières années, par le développement continu des soins opératoires, la reconstruction complète des surfaces articulatoires.
Du fait des risques de lésions vasculaires et cartilagineuses, la reposition immédiate de l’articulation de la hanche s’impose pour tout cas. L’ampleur des lésions et le rêsultat des soins primaires décident de la thérapeutique à suivre. On a le choix entre la conservation ou différentes modalités opératoires. Indépendimment du mode choisi, une extension et une décompression durable de l’articulation sont indispensables.
La coxarthrose posttraumatique fréquente à la suite de ces lésions peut être réduite au minimum par une exacte diagnose radiologique et par une thérapeutique appropriée au cas particulier.
