Endoskopische Adrenalektomie—Indikation, Technik und erste Ergebnisse

Zusammenfassung

Grundlagen:

Endoskopische Operationsmethoden werden auch zunehmend bei Eingriffen an der Nebenniere angewandt.

Methodik:

Anhand eigener erster Ergebnisse und einer Übersicht der relevanten Literatur wird über Indikation und Technik dieser neuen Operationstechnik berichtet.

Ergebnisse:

In der Literatur wurden mehr als 100 Adrenalektomien erfolgreich minimal invasiv durchgeführt. Vier verschiedene endoskopische Zugangswege zur Nebenniere wurden beschrieben— der anteriore transabdominelle Zugang, der transperitoneale Flankenzugang, der retroperitoneoskopische Flankenzugang und der dorsale retroperitoneoskopische Zugang. Der überwiegende Anteil der bisher durchgeführten endoskopischen Adrenalektomien erfolgte wegen Conn-Adenomen oder Inzidentalomen. Allerdings wurden auch Phäochromozytome erfolgreich endoskopisch entfernt. Mangelnde Exposition des Operationsfeldes sowie intraoperative Blutungskomplikationen waren die häufigsten Gründe für die bisher selten berichteten Umstiege zum konventionellen Vorgehen.

Schlußfolgerungen:

Die minimal invasive Adrenalektomie bietet den Patienten die gleichen Vorteile wie andere laparoskopische Operationen: weniger Schmerz, ein besseres kosmetisches Ergebnis, eine kürzere Krankenhausverweildauer und eine kürzere Rekonvaleszenzphase.
