Zusammenfassung
Grundlagen: Das Metastasierungsverhalten von Plattenepithelkarzinomen der Mundhöhle und des Oropharynx ist ein entscheidender Faktor in der Prognose. Im Rahmen der chirurgischen Behandlung der regionären Lymphknoten stehen verschiedene Formen der Dissektion und En-bloc-Resektion zur Verfügung.
Methodik: Basierend auf ausführlicher präoperativer Diagnostik und auf einer Kombinationsbehandlung mit präoperativer Radiochemotherapie wurde seit 1990 ein abgestuftes Konzept zur Lymphknotendissektion — supraomohyoidale Dissektion, konservative und radikale Neck dissection zur Anwendung gebracht und mit mikrovaskulärer Primärrekonstruktion kombiniert. Von Anfang 1990 bis Ende 1994 wurden 79 Patienten nach dieser Vorgangsweise behandelt.
Ergebnisse: Bei 71 zur Verlaufsbeobachtung herangezogenen Patienten waren zum Zeitpunkt der Nachuntersuchung Ende 1996 (Mindestbeobachtungszeitraum 2 Jahre) 16 Patienten an der Tumorerkrankung verstorben (22,5%), 7 Patienten davon an Lymphknotenrezidiven, 9 an Rezidiven im Bereich des Primärtumors. 4 Patienten verstarben an einer zweiten Tumorerkrankung, 2 Patienten an anderer Ursache. Bei den 49 lebenden Patienten bestand zum Zeitpunkt der Nachuntersuchung Tumorfreiheit.
Schlußfolgerungen: Zusammen mit der Kombinationstherapie und der ausführlichen Diagnostik zeigt sich bei dem abgestuften Konzept der Halslymphknotenausräumung gemeinsam mit der mikrovaskulären Primärrekonstruktion keine erhöhte Rate an Rezidiven bzw. Lymphknotenrezidiven. Durch die teilweise schonendere Halsoperation wird die lokale Morbidität verringert, und durch die primäre Wiederherstellung kann die postoperative Lebensqualität verbessert werden.
Summary
Background: Regional metastatic disease of patients with squamous cell carcinomas of the oral cavity and the oropharynx is a decisive prognostic factor. Various types of regional lymph node dissections are used, combined with en bloc resection.
Methods: Based on detailed preoperative diagnosis and combined modality treatment with preoperative radiochemotherapy, a step-by-step concept of lymph node dissection — supraomohyoidal dissection, conservative and radical neck dissection — was combined with microvascular primary reconstruction. Since 1990 until the end of 1994 79 patients were treated and 71 have been evaluated.
Results: With a minimal observation time of 2 years, 16 out of 71 patients (22.5%) died of recurrent tumor until the end of 1996. In 7 patients tumor recurrence occurred in regional lymph nodes, in 9 patients at the site of the primary lesion. 4 patients died of a second malignancy, 2 patients not tumor-associated. The 49 patients alive presented, were without any evidence of disease.
Conclusions: With combined modality treatment and detailed diagnosis for the step-by-step concept of cervical lymph node dissection with preservation of donor vessels for microvascular reconstruction, no increased number of recurrent tumors in regional lymph nodes has been found. By less invasive and radical cervical dissection postoperative morbidity could be lowered and by primary reconstruction quality of live could be improved.
