Zusammenfassung
Grundlagen: In frühen Stadien — Ann Arbor E I1, sogenanntes „Frühlymphom“ — von niedrigmalignen primären Magenlymphomen vom MALT-Typ kann eine lwöchige antibiotische Eradikationstherapie des Helicobacter pylori in 80% zu kompletten Remissionen dieser Tumore führen.
Methodik: Sorgfältiges klinisches Staging und histologisches Grading ist Voraussetzung zur Vermeidung von Therapieversagern in dieser Gruppe. Konservative, nichtchirurgische Behandlungsformen von niedrigmalignen Lymphomen in höheren Stadien oder von hochmalignen Lymphomen sind im Gespräch.
Ergebnisse: Erste Berichte von RTX und CTX weisen ähnliche 5-Jahres-Überlebensraten auf, wie sie von retrospektiven Studien nach chirurgischer Behandlung bekannt sind, und betonen die bessere Lebensqualität der Patienten.
Schlußfolgerungen: Die traditionelle Resektionstherapie des Magenlymphoms als Therapie der „ersten Wahl“ sollte überdacht werden. Bei niedrigmalignen „Frühlymphomen“ ist der Erfolg der „first line“-Hp-Eradikationstherapie abzuwarten. Darüber hinaus ist derzeit eine europäische prospektive randomisierte Studie in Vorbereitung, welche chirurgische und nichtchirurgische Behandlungsweisen vergleicht. Die Ergebnisse werden eine individuell maßgeschneiderte Therapie mit besten Überlebenschancen und hoher Lebensqualität finden helfen.
Summary
Background: Helicobacter pylori eradication therapy has proven to be effective in gaining complete remission of primary low grade gastric lymphoma of MALT type in "early" stage, namely E I1 according to the Ann Arbor system, in about 80%.
Methods: Thorough clinical staging and histological grading is necessary to preclude failure of antibiotic monotherapy. However, different modes of treatment of both, low grade lymphoma in stages ≥ E I2 and high grade lymphoma, with respect to stomach conserving, non-surgical methods are under discussion.
Results: First results of RTX and CTX therapy report 5 years survival rates comparable to surgical treatment and claim better quality of patients’ life.
Conclusions: To date, an European prospective randomized trial comparing surgical with non-surgical treatment modalities is on its way in order to help establishing the most effective therapy as to survival at optimal quality of life for the individual patient.
