Zusammenfassung
Grundlagen: Die akute hyperkalzämische Krise ist auch heute noch eine ernsthafte wenn auch seltene Verlaufsform des primären Hyperparathyreoidismus (pHPT).
Methodik: Aufgrund der Krankenunterlagen von 8 Patienten mit akutem pHPT von 1986 bis 1996 werden klinische Manifestation, mögliche Auslöser und unterschiedliche Behandlungsregime diskutiert. Bei dem krisenhaften pHPT ist die Operation absolut indiziert und die einzige etablierte kausale Therapie. Eine postoperative Letalität von 5% ist in erster Linie auf verzögerte, unzureichende oder zu lange protrahierte Vorbehandlung zurückzuführen.
Ergebnisse: Eine adäquate Rehydratation und forcierte Diurese mit Furosemid bilden die Basis der Vorbehandlung. Das nur kurzfristig kalziumsenkend wirksame Kalzitonin kann unter bestimmten Voraussetzungen durch Bisphosphonate ergänzt werden.
Schlußfolgerungen: Die Operation sollte nach zeitlich limitierter Vorbehandlung von 24 h (bis 48 h) erfolgen. Die Operationsverfahren in der Akutsituation unterscheiden sich nicht von denen der Elektivoperation. Das spezifische Operationsrisiko ist nicht erhöht.
Summary
Background: Even nowadays the hypercalcemic crisis is a severe but rare manifestation of primary hyperparathyroidism (pHPT).
Methods: The clinical appearance, possible triggering factors and different treatment of a hypercalcemic crisis due pHPT, based on patient reports (8 cases from 1986 to 1996), are discussed. In case of an acute primary hyperparathyroidism surgery is absolutely indicated. Surgery is the only established and live saving treatment. A postoperative lethality of about 5% is primarily attributable to delayed, inadequate, or highly protracted pretreatment.
Results: The adequate rehydratation combined with a forced diuresis with furosemid is the basis of the pretreatment. Calcitonin which lowers calcium only for a short period of time can be supplemented under certain circumstances with bisphosphonates.
Conclusions: After an adequate pretreatment surgery should be performed within 24 (to 48) hours. In case of an acute pHPT there is no difference in the method of surgery compared to the elective situation. The specific risks are comparable in both situations.
