Zusammenfassung
Grundlagen: Die distalen Radiusfrakturen werden nach wie vor vorwiegend konservativ behandelt. Eine Vielzahl von ungünstigen Ausheilungsergebnissen nach dieser Behandlungsform verlangt jedoch zu überdenken, bei welchen Frakturformen günstige und bei welchen ungünstige Ergebnisse erwartet werden können. Grundlage für eine therapeutische Empfehlung ist die Kenntnis darüber, worin der Vorteil bzw. der Nachteil einer standardisierten Behandlungsmethode zu sehen ist.
Methodik: In einer retrospektiven Studie über extraartikuläre metaphysäre Radiusfrakturen wird der Einfluß der Fehlstellung auf das klinische Ergebnis untersucht. Das Kollektiv umfaßt 56 junge Erwachsene (Durchschnittsalter 47 Jahre, min. 28 Jahre, max. 64 Jahre). Die Nachuntersuchung erfolgte durchschnittlich 9 Jahre (min. 8 Jahre, max. 12 Jahre) nach der Verletzung. Das radiologische Endresultat wurde einem klinischen Score gegenübergestellt.
Ergebnisse: Als prognostisch relevant (p=0,0025) erwies sich die Verkürzung der Speiche im distalen Radioulnargelenk (DRUG). Verkürzungen bis zu 2 mm ergaben ein gutes klinisches Ergebnis. Bei größeren Verkürzungen war das Resultat schlechter. Nicht signifikant waren die isolierte Abflachung des dorsopalmaren (p=0,299) und/oder des radioulnaren (p = 0,813) Radiusgelenkwinkels.
Schlußfolgerungen: Bei der konservativen Behandlung ist besonders auf den Längenerhalt des Radius zu achten. Nachdem die distale Radiusfraktur vorwiegend mit einer Instabilität im distalen Radioulnargelenk verbunden ist, spielt eine geringe Inkongruenz des DRUG offensichtlich keine so wesentliche Rolle. Die Protrusion des Ellenkopfes führt zu einem Impingement-Syndrom im ulnaren Handgelenkompartiment mit Schmerzen, Funktionsbehinderung und Sekundärfolgen.
Summary
Background: For distal radius fractures conservative treatment is still the therapy of choice. However, one must consider that the fracture type is relevant for the final result. To recommend another treatment pros and cons of standardized conservative treatment have to be evaluated.
Methods: This retrospective study about extraarticular distal radius fractures concerns about the anatomical result and its affect on the final outcome. 56 young adults (mean 41 years; min. 28 years, max. 64 years) were investigated 9 years (min. 8 years, max. 12 years) after their injury.
Results: The radiological results were compared to a clinical score. Shortening of the radius and its sequelaes to the distal radioulnar joint (DRUJ) was prognostic significant (p=0.0025). An ulna plus variance up to 2 mm reported good clinical results. An increased ulna plus situation showed worse results. The dorsopalmar angle (p=0.299) and the radioulnar angle (p=0.813) were not significant.
Conclusions: For distal radius fractures conservative treatment is the measurement of choice. Especially correction of axial alignment at the DRUJ has to be focused during follow up. Incongruence at the DRUJ seems to be of minor importance because of the instability at the DRUJ frequently accompaning distal radius fractures. On the contrary protrusion of the ulna head can lead to a painful ulnocarpal impingement with limited motion.
