Zusammenfassung
Grundlagen: Der Einsatz der Computertomographie (CT) bei Frakturen ermöglicht eine überlagerungsfreie Darstellung der Fragmente. Durch die technische Weiterentwicklung der CT mittels Spiral-Technik, sehr dünnen Schichten (1 mm Schichtdicke) und hochauflösenden Untersuchungs-Modus wird die Frakturdiagnostik weiter verfeinert. Zusätzlich kann die Handgelenks-Arthrographie Kapsel-Band Verletzungen darstellen.
Methodik: Anhand von klinischen Beispielen wird die Wertigkeit der hochauflösenden Spiral-CT und der Handgelenks-Arthrographie zur Abklärung distaler Radiusfrakturen beurteilt.
Ergebnisse: Die hochauflösende Spiral-CT kann aufgrund der kurzen Untersuchungszeit, der exakten überlagerungsfreien Darstellung der Fragmente und der Erstellung eines 3D-Datensatzes inklusive hochwertige 2D-Rekonstruktionen in weiteren Ebenen, 3D-Darstellung sowie Direktaufsicht auf die Gelenkfläche nach elektronischer Exartikulation die distale Radiusfraktur exakt darstellen und klassifizieren.
Durch die Kombination von hochauflösenden Spiral-CT und Arthrographie werden Kapsel-Band Verletzungen eindeutig zugeordnet, aufgrund des Befundmusters werden degenerative von traumatischen Band- und Diskusverletzungen unterschieden.
Schlußfolgerung: Die hochauflösende Spiral-CT Untersuchung nach distaler Radiusfraktur ist die Methode der Wahl zur weiteren Abklärung und Darstellung der komplexen Frakturpathologie.
Durch den zusätzlichen Einsatz der Arthrographie und Arthro-CT werden bei komplexen distalen Radiusfrakturen vorhandene Kapsel-Band Verletzungen dargestellt und aufgrund des Befundmusters traumatische von degenerativen Veränderungen unterschieden.
Summary
Background: Computed tomography (CT) displays distal radius fracture without superimposition. The technical developments of CT with spiral data aquisition, thin slices (1 mm slice thickness) and high-resolution scanning allow even better representation of fracture pathology. Arthrography of the wrist demonstrates additional ligamentous and fibrocartilage lesions
Methods: In a review the value of high-resolution CT and the combination of wrist-arthrography/arthro-CT of distal radius fractures was evaluated.
Results: Due to the short examination time and the exact delineation of the fragments without superimposition as well as the advantages of 3D-techniques enables high-resolution spiral-CT to classify exact complex distal radius fracture. Arthro-CT displays the various ligamentous and fibrocartilage lesions of distal radius fracture to a better advantage than arthrography alone.
Conclusions: High-resolution spiral-CT of the wrist after distal radius fracture represents the method of choice to evaluate the exact fracture pathology. Additional Arthro-CT allows an exact classification and recognition of capsular and ligamentous lesions. Due to the pattern of ligamentous and fibrocartilage lesions it is possible to differentiate between acute traumatic and chronic degenerative alterations.
