Zusammenfassung
Grundlagen: Die Überstreckungsverletzung des Handgelenkes wird, wenn radiologisch keine Frakturen nachgewiesen werden können, oft bagatellisiert. Primär als „Distorsion“ des Handgelenkes abgetan, erweist sie sich im weiteren Verlauf häufig als „okkulte“ Fraktur oder als karpale Bandverletzung, die letztlich zur karpalen Gefügestörung und somit zu einem sehr schwer zu behandelnden Spätschaden führt.
Methodik: An 130 Präparaten wurde eine Überstreckungsverletzung simuliert. Die radiologischen Veränderungen während des Versuches wurden mittels zweier Röntgenbildverstärker aufgezeichnet, die Präparate nach Druckentlastung auf Verletzungen des radiokarpalen und interkarpalen Bandapparates, des Discus ulnocarpalis und auf Schäden der Membrana interossea sowie der Sehnen präparatorisch untersucht. 20 Präparate wurden unter Kompression tiefgefroren und in dieser Position mit dem Kryotom nachKathrein Serienschnitte angefertigt.
Ergebnisse: Ein erster Versuchsablauf hat gezeigt, daß bei Kompression mit Überstreckung des Handgelenkes und Blockierung des proximalen Radioulnargelenkes Frakturen im Unterarmbereich entstehen. Zur Erzeugung distaler Radiusfraktur ist die freie Beweglichkeit beider Radioulnargelenke notwendig.
Durch den identen Untersuchungsablauf ließen sich einerseits isolierte Läsionen des radiokarpalen und interkarpalen Bandapparates und des Diskus, isolierte Frakturen der Karpalia und des distalen Radius sowie auch Kombinationsverletzungen erzeugen. Durch die Überstreckung des Handgelenkes kam es sowohl zu dorsalen als auch zu zentralen und palmaren Frakturformen des distalen Radius.
Schlußfolgerungen: Die karpalen Begleitverletzungen sowie die Stauchungshöhle nach distalen Radiusfrakturen erweisen sich als wesentliche Probleme bei der Behandlung handgelenksnaher Speichenbrüche. Diskutiert werden mögliche Ursachen der karpalen Bandverletzungen und der Verletzungsmechanismus distaler Radiusfrakturen.
Summary
Background: Without radiological evidence of a fracture hyperextension injuries of the wrist may be underestimated. A distorsion trauma of the wrist, which has been determined at initial evaluation, later may reveal an occult fracture or carpal ligament injury. Finally, such injuries may lead to disturbance of the carpal structure and to difficulty in treatment of late defects.
Methods: In 130 specimens hyperextension injuries were simulated. Radiographic changes during the experiment were recorded with 2 X-ray intensifiers. After reduction of pressure the specimens were investigated to determine injuries of the radiocarpal and intercarpal ligaments, of the ulnarcarpal disc, of the interosseus membrane, and of the tendons and tendon sheaths. 20 specimens were deep frozen while under compression and serial cryosectioning with a cryotome according toKathrein was performed.
Results: In a pilot project under compression and with hyperextension of the wrist and blockade of the proximal radioulnar joint, fractures of the forearm occurred. Free mobility of both radioulnar joints is necessary to produce distal radial fractures. Identical course of the experiment helped to produce isolated lesions of the radiocarpal and intercarpal ligaments and of the disc, isolated fractures of the carpal bones and of the distal radius, and combined injuries. Hyperextension of the wrist lead to dorsal, central and palmar fractures of the distal radius.
Conclusions: Carpal ligamentous injuries and cancellous bone defects are the major problems in treatment of distal radial fractures. Pathomechanism of the radial fractures and possible reasons for carpal ligament injuries will be discussed.
