Anal precancers: a challenge for surgeons and pathologists

Abstract

Background: Precancerous changes in the anal area are diagnosed with increasing frequency in the Western world and represent a challenge for surgeons and pathologists.

Methods: Review of literature based on Medline-search dated June 1994 and own literature register comprising 3100 articles on anal anatomy and pathology and related subjects.

Results: Precancerous changes are oftenpart of a multicentric anogenital neoplasia and considerable evidence links them to venereal diseases. Symptoms are vague but risk groups can be identified. A rather conservative surgical approach seems justified, but close follow-up is necessary.

Conclusions: A close collaboration between clinicians and pathologists is mandatory in treating precancerous changes. A recommendable procedure for this is outlined.

Zusammenfassung

Grundlagen: Präkanzeröse Veränderungen der Analregion werden in der westlichen Welt mit zunehmender Häufigkeit diagnostiziert und stellen eine Herausforderung für Chirurgen und Pathologen dar.

Methodik: Literatur-Review, beruhend auf einer Medline-Abfrage vom Juni 1994 sowie auf dem eigenen Literaturregister, das 3100 Artikel über anale Anatomie, Pathologie und assoziierte Bereiche umfaßt.

Ergebnisse: Präkanzeröse Veränderungen sind oft Teil einer multizentrischen anogenitalen Neoplasie. Es gibt gewichtige Hinweise auf eine Verbindung zu Geschlechtskrankheiten. Symptome sind unspezifisch, aber Risikogruppen können definiert werden. Eine eher zurückhaltende chirurgische Einstellung erscheint gerechtfertigt, bedingt jedoch eine exakte Kontrolle der Patienten.

Schlußfolgerungen: In der Behandlung von präkanzerösen Veränderungen ist die enge Zusammenarbeit zwischen Klinikern und Pathologen unbedingt erforderlich. Ein diesbezüglich empfehlenswertes Vorgehen wird beschrieben.
