Zusammenfassung
Grundlagen: Die perkutane Diskektomie (PDE) wurde erstmals 1975 vonHijikata beschrieben. Es werden in dieser Studie die Operationsergebnisse der seit 1989 an der neurochirurgischen Abteilung der Landesnervenklinik Salzburg durchgeführten perkutanen Diskektomien retrospektiv analysiert.
Methodik: Die perkutanen Diskektomien wurden über einen dorsolateralen Zugang unter Verwendung des Instrumentariums von Aeskulap® durchgeführt. Die Patienten wurden klinisch nachuntersucht, wobei der durchschnittliche Nachuntersuchungszeitraum 21,3 Monate betrug.
Ergebnisse: Es wurde insgesamt bei 41 Patienten die PDE durchgeführt. 36,6% der Patienten waren postoperativ beschwerdefrei; 31,7% der Patienten erreichten ein gutes und 7,1% ein befriedigendes Operationsergebnis. Bei 10 Patienten war wegen fehlendem Therapieerfolg eine interlaminäre Diskusextraktion notwendig. Intra- und postoperative Komplikationen traten nicht auf.
Schlußfolgerungen: Bei der perkutanen Diskektomie handelt es sich um eine ergänzende Operationsmethode in der Bandscheibenchirurgie, die für wenige Patienten die Therapie der Wahl darstellt. Die Operationsergebnisse sind bei strenger Indikationsstellung den Ergebnissen der interlaminären Diskusextraktion vergleichbar.
Summary
Background: Percutaneous discectomy was described in 1975 byHijikata. In this retrospective study the postoperative results of percutanous discectomy, which was practized since 1989 at the Neurosurgical Department of the Landesnervenklinik Salzburg are evaluated.
Methods: Percutaneous discectomy was practized by a dorsolateral access using the Aeskulap discectomy set. On the average clinical examination of patients was performed 21,3 months after operation.
Results: On 41 patients PDE was practized. In 36.6% of cases a total relief of symptoms could be found. 31.7% of patient had good and 7.1% had satisfying results. Because of persisting symptoms on 10 patients an interlaminar discectomy was necessary.
Conclusions: Percutanous discectomy is a completing method in disc surgery for a small part of the patients. Severe indication would be necessary to achieve results which are compareable to interlaminar discectomy.
