Zusammenfassung
Grundlagen: Wenngleich die iatrogene Verschleppung von Tumorzellen eine seit langem bekannte Tatsache ist, ist die Entstehung von Implantationsmetastasen in der Literatur als selten beschrieben.
Methodik: Bei einem 53jährigen männlichen Patienten wurde der Weichteildefekt nach Resektion eines rezidivierenden Plattenepithelkarzinoms im Kieferwinkelbereich mit einem gestielten M.-pectoralis-major-Myokutanlappen gedeckt.
Ergebnisse: 6 Monate nach der Tumoroperation mußte trotz Einhaltung aller üblichen intraoperativen Sicherheitsvorkehrungen an der Entnahmestelle des Pektoralislappens eine Implantationsmetastase beobachtet werden. Diese Eigenbeobachtung war Anlaß, diese selten beschriebene Komplikation zu dokumentieren, einen Literaturvergleich anzustellen sowie weitere Vorsichtsmaßnahmen zu diskutieren.
Schlußfolgerungen: Bei Verdacht auf erhebliche intraoperative Tumorzellfreisetzung, erscheint vor allem bei Patienten mit schlechter Abwehrlage (z. B. durch Malnutrition und aggressive Therapie) neben den üblichen Vorsichtsmaßnahmen eine zusätzliche lokale Anwendung von zytotoxischen Substanzen oder die intraoperative systemische Verabreichung von Zytostatika überlegenswert.
Summary
Background: Iatrogenic implantation of tumor cells is a well recognized but rarely reported phenomenon.
Methods: In a 53-year old male patient a recurrent squamous cell carcinoma was resected in the area of the angle of the mandible.
Results: The resulting soft tissue defect was covered with a pedicled pectoralis major myocutaneous flap. 6 months after the tumor resection an implantation metastasis had to be diagnosed at the donor site of the pectoralis major muscle flap, although all surgical principles had been respected intraoperatively. This personal observation provoked a documentation of this rare complication of ablative tumor surgery in a case report. A research in literature was done and further means to prevent such complications are discussed in this paper.
Conclusions: In patients with depression of resistance (malnutrition, aggressive therapy) intraoperatively application of a caustic agent or chemotherapy could be performed if massive seeding of tumor cells in the adjacent surgical field is suspected.
