Zusammenfassung
Grundlagen: Das Ziel der adjuvanten Radiotherapie beim Rektumkarzinom liegt in der Senkung der Lokalrezidivrate, um dadurch zu einer höheren Heilungsrate und Überleben zu führen.
Methodik: Anhand wichtiger internationaler Studien wird der Stellenwert der Radiotherapie dargestellt.
Resultate: Die Verträglichkeit der adjuvanten Radiotherapie ist in der Regel sehr gut; sie führt weder bei niedriger (5 bis 20 Gy in 1 bis 2 Wochen), noch bei höherer Dosierung (45 bis 50 Gy in 5 bis 6 Wochen) zu unakzeptablen Nebenwirkungen. Insbesondere läßt sich keine Zunahme von Anastomosendehiszenzen feststellen. Dagegen ist die simultane Verwendung von Radiotherapie und systemischer Chemotherapie mit signifikanter Toxizität behaftet. Obwohl die meisten Studien von einer Reduktion der Lokalrezidivrate berichten, ist die Verbesserung bezüglich des Überlebens ausgeblieben; nur bei selektierten Patientenpopulationen konnte ein längeres Überleben festgestellt werden.
Schlußfolgerungen: Prospektive randomisierte Studien um prognostische Faktoren, den Wert der adjuvanten Radiotherapie und ihren Einfluß auf die Heilung sind unumgänglich und sollten auf möglichst breiter Basis durchgeführt werden.
Summary
Background: The aim of adjuvant radiotherapy in treatment of rectal cancers is primarily to decrease the rate of local recurrences and thereby increase disease free survival and overall survival.
Methods: Reviewing relevant international studies the importance of radiotherapy is described.
Results: The tolerance of adjuvant radiotherapy is generally good; unacceptable side effects are neither seen at low-dose (5 to 20 Gy in 1 to 2 weeks), nor at higher doses of radiation (45 to 50 Gy in 5 to 6 weeks). No increase in leakage frequency at the site of the anastomosis has been recorded. When chemotherapy is administered simultaneously with irradiation, the overall toxicity is increased significantly. Although most studies report a decrease in local recurrence rate, following adjuvant radiotherapy, an increase in survival has only been seen in selected subgroups of patients.
Conclusions: Prospective randomized trials considering prognostic factors, are warranted, to be able to establish the optimal adjuvant therapy in rectal cancers.
