Zusammenfassung
Grundlagen: Eine kleine Patientengruppe mit Rektumkarzinom ist bei Diagnosestellung für einen kurativen Eingriff nicht geeignet, benötigt aber eine symptomatische, palliative Therapie.
Methodik: Zur Palliation wurde der Neodym-YAG-Laser und/oder die endoluminale Brachytherapie (Afterloading) bei 63 Patienten angewandt.
Ergebnisse: Bei 92 % der Patienten erreichten wir durch Neodym-YAG-Lasertherapie eine primäre Desobliteration, bei 85 % war auch im weiteren Verlauf keine Kolostomie notwendig. Schwere Komplikationen traten in 4,7 %, leichte in 14,2 % der Fälle auf. Die therapiebezogene Letalität war 0 %, die durchschnittliche Überlebenszeit betrug 10,2 Monate. 12 Patienten erhielten im Anschluß an die Lasertherapie eine endoluminale Brachytherapie, wodurch die Effektivität dieser Palliativmethode noch erhöht wurde.
Schlußfolgerungen: Die Forderungen an eine optimale Palliativmaßnahme sind hohe Effektivität, rasche Beseitigung der Symptome, kurze Hospitalisierung, hohe Lebensqualität, geringe Morbidität und Letalität. Die Kombination der Lasertherapie mit endoluminaler Brachytherapie kann dieses Ziel noch verbessern.
Summary
Background: At the time of diagnosis a small number of patients with rectal cancer cannot be treated for cure and need a symptomatic, palliative treatment.
Methods: For palliation the Neodym-YAG-laser and/or the endoluminal brachytherapy (afterloading) was used in 63 patients.
Results: With the Neodym-YAG-lasertherapy a primary desobliteration was achieved in 92 % of the patients, 85 % did not need colostomy during the further course. Serious complications occurred in 4,7 % minor in 14,2 % of all cases. Lethality, caused by therapy, was 0 %, the mean survival time was 10,2 months. Following the lasertherapy 12 patients received endocavitary radiation, obviously improving the efficiency of this palliative modality.
Conclusions: The demands for an optimal palliation are high effectiveness, fast elimination of symptoms, short hospitalisation, high quality of life, low morbidity and lethality. A combination of lasertherapy and endoluminal brachytherapy can improve this aim.
