Zusammenfassung
Grundlagen: Die koloanale Anastomose zur Behandlung tiefsitzender Tumoren des Rektums wurde hinsichtlich Komplikationsrate, onkologischem und klinisch-funktionellem Ergebnis untersucht.
Methodik: Prinzip der Operation ist die Wiederherstellung der Kontinuität nach der von abdominell vorgenommenen Rektumresektion mittels transanal durchgeführter Anastomose zwischen Colon descendens und Linea dentata.
Ergebnisse: Bei 86 Patienten mit einem Rektumtumor wurde eine koloanale Anastomose durchgefürhrt. Endoskopisch betrug der Abstand ab ano 6,1±1,8 cm (\( \bar x \pm SD \)). Die chirurgische Komplikationsrate ist ähnlich der nach vorderer Rektumresektion. Nach einer medianen Beobachtungszeit von 42,8 Monaten beträgt die Lokalrezidivrate 13,3%. Die Sphinkterfunktion war für alle Patienten zufriedenstellend. Die in den ersten postoperativen Monaten störende hohe Stuhlfrequenz reduzierte sich innerhalb eines Jahres auf durchschnittlich 3 Stühle pro Tag.
Schlußfolgerungen: Die koloanale Anastomose ist eine hinsichtlich onkologischem und funktionellem Ergebnis akzeptable Alternative zur synchronen Rektumexstirpation in der Behandlung tiefsitzender Rektumtumoren.
Summary
Background: Coloanal anastomosis as a surgical procedure for low rectal tumors was analyzed in regard of complication rate, oncological and functional results.
Methods: The principle of the operation is to restore large bowel continuity after abdominal rectal resection by transanal approach, performing an anastomosis between descending colon and the dentate line.
Results: In 86 patients with rectal tumors coloanal anastomosis was performed. Endoscopic tumor distance from the anal verge was 6.1±1.8 cm (\( \bar x \pm SD \)). The complication rate was comparable to that after anterior resection with conventional anastomosis. After a mean observation time of 42.8 months local recurrence rate was 13.3%. Sphincter function was acceptable for all our patients. The high frequency of defecations during the first months after surgery decreased within one year to a mean of 3 stools per day.
Conclusions: Coloanal anastomosis as a treatment for low rectal tumors is an acceptable alternative to abdomino-perineal excision regarding oncological and functional results.
