Minimally invasive cardiac surgery

Zusammenfassung

Grundlagen: „Minimal invasiv” bedeutet in der Herzchirurgie Minimierung des chirurgischen Operationstraumas durch verkleinerte Inzision und Vermeidung des systemischen Traumas durch Vermeidung der extrakorporalen Zirkulation. Die angewandten Techniken befinden sich in stürmischer Entwicklung, eine endgültige Definition des Begriffes „minimal invasiv” ist noch nicht gegeben.

Methodik: Die meisten herzchirurgischen Eingriffe sind heute in weniger invasiver Technik möglich, eine Selektion der Patienten für bestimmte Operationstechniken ist erforderlich. Die Anwendbarkeit dieser Techniken ist nur in einem beschränkten, noch nicht klar definierten Prozentsatz der Patienten gegeben.

Ergebnisse: Nach Überwindung der „learning curve” sind die Ergebnisse mit denen der konventionellen Operationen vergleichbar. Vorteile durch minimal invasive Techniken sind durch kontrollierte Studien nicht nachgewiesen.

Schlußfolgerungen: Indikationen für minimal invasive Herzoperationen sind situationsangepaßt zu stellen. Goldstandard ist die Operation in konventioneller Technik mit extrakorporaler Zirkulation.

Summary

Background: Minimally invasive cardiac surgery is defined by reduction of the surgical trauma using miniincisions, as well as reduction of the systemic trauma induced by use of extracorporeal circulation. The various techniques are evolving rapidly. The term “minimally invasive” in cardiac surgery still awaits further definition.

Methods: Most cardiac surgical operations can be performed less invasively. Operative techniques should be selected individually. A major, not clearly defined percentage of patients has to be operated in the conventional way.

Results: After overcoming the learning curve results are comparable to conventional technique. Advantages by minimal invasive techniques may be biased and are not documented by prospective studies.

Conclusions: Indications for minimal invasive cardiac operations should be drawn according to specific situations if an advantage is awaited. Gold standard still is the conventional technique employing extracorporeal circulation.
