Summary
Background: The dismal prognosis of patients with an esophageal carcinoma can only be improved by R0 resection. However, this condition is very difficult to be achieved in cases with locally advanced (pT4)-tumors.
Methods: This paper reviews the results of diagnostic and neoadjuvant therapeutic modalities in patients with esophageal carcinoma that invades adjacent organs.
Results: Endosonography (7,5 and 12 MHz frequencies) is with an accuracy rate of approximately 86% actually the most accurate method for staging esophageal carcinoma. Randomized clinical trials indicate that neoadjuvant radiotherapy does not increase the resection rate or prolong survival (5-year survival rate 9.5 to 16%) compared to surgical resection alone. Data from several series suggest that preoperative cisplatin-based chemotherapy is effective, but it is associated with a high perioperative morbidity (>45%) and mortality (>15%) rate. Neoadjuvant radio-chemotherapy appears to improve the prognosis only in those patients with objective clinical or histopathologic response. Due to a high postoperative mortality, combined resection of the tracheobronchial tree and aorta is considered too invasive and therapeutically not worthwhile. When patients are appropriately selected, a combined pulmonary (including lobectomy), pericard and esophageal resection is a relatively safe procedure. The survival curve seems to be better in these patient groups.
Conclusions: Patients with locally advanced esophageal carcinoma should undergo neoadjuvant treatment. This should be performed on appropriately selected cases and in experienced centers within the context of clinical trials. The available data on the role of neoadjuvant radio-chemotherapy are not conclusive and a recommendation can not yet be given. Multivisceral resection should be performed only if complete tumor resection can be anticipated and only in those patients who respond to neoadjuvant treatment modalities.
Zusammenfassung
Grundlagen: Eine Verbesserung der Prognose von Patienten mit einem Ösophaguskarzinom kann nur durch das Erreichen einer R0-Resektion erreicht werden, was bei Vorliegen eines pT4-Karzinoms, besonders bei suprabifurkaler Lokalisation, schwer zu gestalten ist.
Methodik: In dieser Arbeit stellen wir anhand eigener Erfahrungen und Bewertung der Literatur Ergebnisse diagnostischer und multimodaler sowie operativer Therapieverfahren bei Patienten mit einem T4-Ösophaguskarzinom dar.
Ergebnisse: Die flexible Endosonographie mit hochfrequenten Transducern (7,5 bis 12 MHz) ist hinsichtlich der Feststellung eines T4-Tumors zur Zeit mit einer Treffsicherheit von 86% die sensitivste Methode. In verschiedenen Phase-II- und III-Studien zur neoadjuvanten Radiotherapie zeigte sich keine Steigerung der Resektionsrate und kein Einfluß auf die 5-Jahres-Überlebensrate (9,5 bis 16%), so daß dieses Verfahren als alleinige Methode weitestgehend verlassen wurde. Eine auf Cisplatin basierende neoadjuvante Chemotherapie ist als prinzipiell wirksam anzusehen. Diese ist jedoch mit einer sehr hohen perioperativen Morbiditäts- (>45%) und Letalitätsrate (>15%) assoziiert. Von einer neoadjuvanten Radiochemotherapie profitieren nur Responder. d. h. diejenigen Patienten, die mit einem eindeutigen Down Staging angesprochen haben. Demgegenüber haben Patienten ohne Ansprechen auf die Vorbehandlung trotz nachfolgender R0-Resektion oder Patienten mit inkompletter Resektion nur eine kurze Lebenserwartung. Die wenigen zur Verfügung stehenden Erfahrungen zum multiviszeralen Eingriff ergeben, daß die Ergebnisse nach Mitresektion des Tracheobronchialsystems bzw. der Aorta mit einer sehr hohen postoperativen Letalität behaftet sind. Bei der Mitentfernung anderer Organe, wie Lunge und Perikard, scheint die Langzeitprognose etwas günstiger zu sein.
Schlußfolgerungen: Beim T4-Ösophaguskarzinom ist der Einsatz multimodaler Therapiekonzepte zu fordern. Diese sollte an streng selektierten Patienten und in entsprechenden Zentren im Rahmen klinischer Studien durchgeführt werden. Welche neoadjuvante Therapieform die günstigsten Ergebnisse bietet, kann heute noch nicht beantwortet werden. Bei der Entscheidung zu einem multiviszeralen Eingriff muß eine R0-Resektion erreichbar erscheinen, und der Patient auf eine Vorbehandlung objektiv mit einer deutlichen Tumorverkleinerung angesprochen haben.
