Summary
Background: Minimally Invasive Direct Coronary Artery Bypass (MIDCAB) requires a perioperative management that is different from the traditional approach. Control of heart rate is essential for facilitating surgery and protecting the heart against the ischemic insult during occlusion of the target vessel. Absence of cardiopulmonary bypass (CPB) associated organ dysfunction and decreased surgical trauma allows rapid recovery from anesthesia and early extubation.
Methods: Little has been published concerning optimal patient management with MIDCAB procedures. Therefore we present our current perioperative anesthetic approach with these patients.
Results: Acceptable systemic hemodynamics, excellent surgical conditions and optimal patient comfort can be achieved by an anesthetic technique which consists of the combination of thoracic epidural anesthesia with the use of intravenous remifentanil. Meticulous maintenance of body temperature and postoperative pain control are of paramount importance.
Conclusions: As with other surgical procedures, a specific anesthetic management contributes to optimal patient care. Early extubation and transfer to an observation ward may be feasible in selected patients.
Zusammenfassung
Hintergrund: Die minimal invasive Koronarchirurgie am schlagenden Herzen (minimally invasive direct coronary artery bypass [MIDCAB]) erfordert ein perioperatives anästhesiologisches Vorgehen, das sich vom Management des traditionellen herzchirurgischen Patienten unterscheidet. Intraoperativ ist eine Bradykardie wünschenswert, um das chirurgische Vorgehen zu erleichtern und eine günstige Ausgangssituation der myokardialen Sauerstoffbilanz für die Klemmphase des Zielgefäßes zu erreichen.
Methodik: Da nur wenig über ein optimales perioperatives Management bei MIDCAB-Patienten veröffentlicht wurde, stellen wir unser derzeitiges anästhesiologisches und intensivmedizinisches Vorgehen bei diesen Patienten vor.
Ergebnisse: Unser anästhesiologisches Verfahren besteht in der Kombination aus einer thorakalen Epiduralanästhesie und der intravenösen Verabreichung von Remifentanil, einem neuen, ultrakurz wirksamen Opiat. Bei stabiler Hämodynamik werden dadurch exzellente chirurgische Bedingungen bei optimalem Patientenkomfort gewährleistet. Um eine rasche und sichere Extubation der Patienten zu ermöglichen, ist besonderer Wert auf die Vermeidung von Hypothermie und auf eine gute postoperative Analgesie zu legen.
Schlußfolgerungen: MIDCAB erfordert ein differenziertes anästhesiologisches Vorgehen, das eine frühzeitige Extubation und Verlegung ausgewählter Patienten erlaubt.
