Zusammenfassung
Grundlagen: Die Einführung der Mikrochirurgie ermöglichte die freie Transplantation von Dünndarmsegmenten zur Kontinuitätsherstellung nach Tumorresektion des oberen aerodigestiven Traktes. Während der letzten 8 Jahre wurde an der Universitätsklinik Wien bei 105 Patienten der Hypopharynxbzw. obere Ösophagusabschnitt mit insgesamt 107 Dünndarmtransplantaten wiederhergestellt.
Methodik: Die Gefäße der Dünndarmsegmente werden nach der Entnahme mit kalter Eurocollins-Lösung schwerkraftperfundiert und mit mikrochirurgischer Nahttechnik an entsprechende Halsgefäße anastomosiert. Bei Wiederherstellung im Mundbereich wird der Dünndarm (als Patch) vor Anlegen der Mikroanastomosen eingenäht, bei Wiederherstellung der Sprache oder des Schluckaktes (als Rohr) danach. Diese unterschiedliche Vorgangsweise wird gewählt, um im letzteren Fall die autonome Peristaltik des Transplantats durch kurze Ischämiezeit zu erhalten.
Ergebnisse: Bei den meisten Patienten bestand als prädisponierender Faktor im Rahmen der Grundkrankheit ein hohes Lebensalter, starker Alkohol- und Nikotinkonsum. Im Gesamtkollektiv traten 21 Komplikationen auf, die aber in keiner Relation zu den beschriebenen Risikofaktoren standen. Diese werden im Detail aufgezeigt und analysiert. In 8.5% (9/107) traten Transplantatnekrosen auf. 2 Patienten hatte eine Zweittransplantation. Selbst bei älteren Patienten zeigte diese relativ aufwendige Operationstechnik gute Resultate.
Schlußfolgerungen: Wichtig für den Erfolg dieser Eingriffe ist eine intensive interdisziplinäre Kooperation und ein engmaschiges postoperatives Monitoring der Patienten.
Summary
Background: Since the development of microsurgical techniques transplantation of the jejunum for reconstruction of defects of the upper aerodigest tract is possible. During the last 8 years 105 patients with defects of the hypopharynx or upper esophagus were reconstructed with 107 free jejunal transfers at the University of Medicine, Vienna.
Methods: The vessels of the jejunal segments have been anastomosed with microvascular techniques to the vessels of the neck after perfusion with cold Eurocollins-solution. For closure of defects in the mouth the jejunal graft (as a patch) is sutured in before the vessels are anastomosed. For reconstruction of the voice or the digestive tract (as a tube) the vascular anastomoses were done immediately for keeping ischemic time short and herewith preventing damage to autonomic peristalsis of the transplanted jejunum.
Results: Predisposing to the primary disease most of the patients suffered of heavy alcohol and nicotin abuse and were of advanced age. Among the number of our patients we had 21 complications. Those were not correlated to the predisposing factors. The complications are listed and analyzed. In 8,5% (9/107) necrosis of the graft occurred. 2 patients had a 2nd transplantation. This extensive operative procedure had good results even in elderly patients.
Conclusion: A good interdisciplinary cooperation and a close postoperative monitoring is essential for achieving good results.
