Complete loss of the anus and rectum due to crushing injury to the pelvis-treatment using a reversed ileal loop

Summary

A case in which a crushing injury to the pelvis resulted in complete loss of the anorectum, together with the levatores ani, is reported. Management of the patient, particularly the use of a reversed loop of ileum as a replacement for the anal sphincter and rectum, is described. Reports in the literature of crushing injuries to the pelvis which caused avulsion of the rectum are noted, and an explanation of the mechanism likely to be involved in avulsion and expulsion of the rectum is given.
