Zusammenfassung
Grundlagen
Die Korrektur der Pulmonalatresie mit Ventrikelseptumdefekt (PA-VSD) und hypoplastischen oder fehlenden zentralen Pulmonalgeäßen benötigt üblicherweise mehrere chirurgische Engriffe. In vielen Fällen sind durch die Standardzugänge nicht alle zur Korrektur notwendigen Strukturen erreichbar.
Methodik
Der bei uns vorwiegend im Rahmen der bilateralen Lungentransplantation angewendete Zugang der bilateralen transsteralen Thorakotomie ergibt eine sehr gute Darstellbarkeit beider Hillustrukturen, des herzens und beider Pleuraräume und scheint daher ideal für die einzeitige Korrektur auch komplexer multifokale Lungenversorgungsvarianten. Wir haben diesen Zugang bei 6 Patienten angewendet.
Ergebnisse
In allen Fällen waren sämtliche relevanten Strukturen trotz Voroperationen über eine Inzision gut darstellbar. Ein Patient verstarb perioperativ (Operationsmortalität 16,7%). Die Sauerstoffsättigung konnte von 76% (65 bis 88%) auf 96% (91 bis 99%) angehoben werden.
Schlußfolgerungen
Die quere transsternale Thorakotomie erlaubt es, auch komplexe Formen der PA-VSD in einer Operation und durch eine singuläre Inzision zu korrigieren.
Summary
Background
Generally multiple surgical interventions are necessary to correct pulmonary atresia with ventricular septal defect (PA-VSD) and hypoplastic or discontinuous intrapericardial pulmonary arteries. In many cases not all anatomical structures necessary for correction are accessable through a single standard incision.
Methods
We learned about the optimal exposure of hilar structures, the heart and both pleural spaces achieved by transverse thoracosternotomy as we used it routinely in our lung transplant program. This incision seems ideal for one stage correction of complex multifocal aortopulmonary blood supply. We thus used this incision in 6 patients.
Results
In all cases the relevant structures were well accessable through a single incision, despite previous thoracotomies. One patient died perioperatively (operative mortality 16.7%), oxygen saturation increased from 76% (65 to 88%) to 96% (91 to 99%).
Conclusions
The transverse thoracosternotomy is a favourable incision for one stage correction of complex forms of PA-VSD.
