Hintergrund
Die akute Pankreatitis wird unabhängig von den Ursachen und vom Schweregrad zunächst konservativ behandelt.
Methodik
Vorliegende Arbeit gibt eine Übersicht über die Möglichkeiten und Grenzen der konservativen Therapiemaßnahmen.
Ergebnisse und Schlußfolgerungen
Da bis heute keine wirksame Therapie für den zugrundeliegenden Entzündungsprozeß bekannt ist, liegt der Schwerpunkt der Maßnahmen auf der Erkennung und Behandlung häufiger Ursachen (Gallengangstein) und Komplikationen sowie auf intensivtherapeutischen Maßnahmen bei schwerer nekrotisierender Pankreatitis. Da die Komplikationen oft schwer vorhersehbar sind, sollten Patienten mit nachgewiesener Nekrose auf einer Intensivstation interdisziplinär überwacht werden. Während zunächst die Prophylaxe des Volumenmangelschocks im Vordergrund steht, treten später vor allem septisch-toxische Komplikationen auf. Abhängig vom Zustand des Patienten sind das Legen einer Nasensonde, die Gabe von Insulin und eine ausreichende Schmerztherapie notwendig. Beim Auftreten eines Lungenversagens sollte rechtzeitig beatmet werden. Die Frage einer Antibiotikaprophylaxe wird zur Zeit in klinischen Studien überprüft; auf jeden Fall sollte man bei Cholagitis und Hinweisen für eine Bakteriämie antibiotisch behandeln. Die Gabe von Proteasehemmern und die medikamentöse Hemmung der Pankreassekretion haben sich in den klinischen Studien nicht als wirksam erwiesen. Eine ERCP sollte bereits bei Verdacht auf einen Gallengangstein durchgeführt werden. Eine Papillotomie sollte bei Vorliegen von Gallengangsteinen oder Sludge erfolgen.

Background
Primary treatment of patients suffering from acute pancreatitis is conservative, irrespective of its etiology and initial severity.
Methods
This review analyzes the potential and limitations of the current conservative approaches.
Results and Conclusions
There is no specific therapy for treating the underlying disease process. Thus the current approach involves supportive care, elimination of bile duct stones, and treatment of complication. Since complications may develop at any time, patients with moderate or severe disease should be admitted to an intensive care unit for interdisciplinary observation. Basic therapy should include fasting, replacement of volume and electrolytes, as well as adequate analgesia. Depending on the patients’ condition, nasogastric suction and insulin therapy may become necessary. Patients with acute pulmonary failure require early mechanical ventilation. The prophylactic use of antibiotics is currently studied; antibiotics should be given when cholangitis or bacteremia are present. The use of protease inhibitors and drugs capable of inhibiting pancreatic exocrine secretion has not proved effective in clinical trials. Early ERCP should be performed when bile duct stones are suspected. Papillotomy should be carried out only when calculi or sludge are present.
