Long term results following early gastric cancer

Zusammenfassung

Grundlagen

Aufgrund der ausgezeichneten prognostischen Früh- und Langzeitergebnisse des Magenfrühkarzinoms und der in der Literatur angegebenen diskrepanten chirurgischen Therapieempfehlungen analysierten wir in unserem eigenen Krankengut, welche prognostischen Faktoren für die Überlebenswahrscheinlichkeit entscheidend sind.

Methodik

Von 1966 bis 1990 wurden 91 Patienten mit der Diagnose Magenfrühkarzinom an der I. Chirurgischen Universitätsklinik Wien operiert und Klinisch nachuntersucht.

Ergebnisse

Die postoperative Morbidität betrug 11%, die Letalität 4%. Die kumulative Gesamtüberlebensrate nach 10 und 20 Jahren betrug 54,4% und 34,6%. Die Gegenüberstellung mit den vergleichbaren alters- und geschlechtsspezifischen rohen Sterbewahrscheinlichkeiten der österreichischen Normalbevölkerung ergab keinen Unterschied. Tumorgröße (≤25 mm>) und Infiltrationstyp (Mukosa- vs. Submukosakarzinom) zeigten in der univariaten Analyse einen statistischen Unterschied. Im Regressionsmodell nach Cox fanden sich Tumorgröße, Lymphknotenstadium und Tumorgrading als unabhängige prognostische Faktoren.

Schlußfolgerungen

Als chirurgische Konsequenz und Therapieempfehlung ist eine karzinomgerechte Magenresektion mit Lymphknotenstaging indizierf0.
