Zusammenfassung
Die Fortschritte in der pädiatrischen Onkologie beruhen auf der Erkenntnis, daß eine sinnvolle Kombination von verschiedenen Therapiemodalitäten das beste Behandlungsergebnis liefert. Während neuere Operationstechniken und Bestrahlungsverfahren die lokale Tumorkontrolle sichtbar zu verbessern imstande sind, vermag die adjuvante Anwendung aggressiver Polychemotherapieschemata das Problem der Mikrometastasierung in der Mehrzahl der Fälle zu lösen.
In den vergangenen Jahren konnte gezeigt werden, daß eine schon vor der geplanten chirurgischen Intervention eingesetzte Chemotherapie einen immer bedeutenderen Stellenwert einnimmt. Je nach Art und Zeitpunkt der Verabreichung unterscheidet man präoperative, neoadjuvante und Sandwich-Chemotherapie als wesentliche Bestandteile moderner Therapieverfahren. Die präoperative Chemotherapie wird bei Tumoren mit großer Ausdehnung nach bioptisch gesicherter Diagnose angewendet und/oder bei Infiltration lebenswichtiger Organe, wenn eine primäre Tumorresektion aus operationstechnischen Gründen und/oder ohne Verstümmelung nicht möglich ist. Zunehmend erscheint sie auch bei kleineren Tumoren sinnvoll, um die Response-Kinetik auf die gewählte zytostatische Therapie zu evaluieren.
Neben dem Vorteil der Tumorverkleinerung und damit der Verbesserung der Operabilität bietet sie die Möglichkeit einer Verkürzung der postoperativen Therapie sowie Verringerung der Behandlungsintensivität. Nach erfolgter Chemotherapie erstreckt sich die chirurgische Maßnahme oft nur mehr auf die Entfernung einer Narbenplatte oder die Suche nach verbliebenen Tumorzellen. Dies kann als Prognosekriterium herangezogen werden.
Summary
The progress in pediatric oncology is mainly based on a coordinated multimodal treatment approach. New techniques in the fields of surgery and radiation have been able to improve local tumor control. The adjuvant application of polychemotherapy has overcome successfully the systemic spread of tumor cells in the mamority of cases. During the past years the preoperative administration of cytostatics has been established as an essential part of modern therapy strategies. Terms like preoperative, neoadjuvant or sandwich-chemotherapy have been introduced.
The preoperative chemotherapy is most important in patients with large primary tumor mass or infiltration of adjacent organs, if a primary tumor resection is technically not possible or would lead to mutilation. In such a case it is essential to establish the diagnosis bioptically prior to the start of chemotherapy. Another importance of preoperative chemotherapy is the evaluation of the response-kinetic to the applied cytostatic treatment.
The advantages of preoperative chemotherapy are the reduction of tumor size and thus a better operability. Further the duration and intensity of chemotherapy may be decreased. The vitality of the surgically removed tumor tissue and the amount of persistent tumor mass after preoperative chemotherapy are important prognostic parameters.
