Congenital duodenal obstruction

Zusammenfassung

Zwischen 1971 und 1990 wurden 69 Kinder mit angeborenen Duodenalverschlüssen behandelt. Die 20jährige Übersicht zeigt, daß durch Verbesserungen der Diagnostik, insbesondere durch Einsatz der Sonographie und Verfeinerung der Operationsverfahren die durchschnittliche Letalität auf 14,7% gesunken ist, die heute im wesentlichen nur noch von Begleitfehlbildungen und Unreife der Neugeborenen bestimmt wird. Mit 19% lag die Relaparotomierate noch verhältnismäßig hoch. Diese könnte durch Einsatz moderner Verfahren, wie Endoskopie und Laserchirurgie, weiter gesenkt werden.

Summary

For patients with congenital duodenal obstructions the prognosis has continued to improve. Long-term investigations provide detailed information on different anatomic forms of this malformation. For the 69 children aged 0 to 18 years, treated between 1971 and 1990 the mortality was still 14.7% caused by prematurity and associated anomalies. Every 5th patient had to be reoperated. Operative correction of congenital duodenal obstructions has been streamlined as to anatomic and functional demands. Endoscopy and laser surgery may be useful tools to achieve further improvement.
