Zusammenfassung
Grundlagen
Es war das Ziel dieser Studie, altersabhängige Unterschiede im röntgenologischen Aussehen und in der AO-Klassifikation bei Patienten mit konservativ behandelten Radiusfrakturen an typischer Stelle zu beschreiben.
Methodik
In einer Beobachtungsstudie mit retrolektiver Datensammlung wurden die Röntgenbilder aller Patienten (n=334), die im Unfallkrankenhaus Linz im Jahre 1988 konservativ behandelt worden waren, untersucht. Es wurde eine Testgruppe von Patienten, die 60 Jahre alt und älter waren, mit einer Kontrollgruppe von Patienten, die 59 Jahre alt und jünger waren, verglichen.
Ergebnisse
Es wurden 337 Radiusfrakturen loco typico bei 334 Patienten beobachtet, der Altersmedian lag bei 59 Jahren. In der AO-Klassifikation waren in der Testgruppe A.3 Frakturen signifikant häufiger (Chi-Quadrat-Test, p-0,01). Dorsale Segmentinstabilitäten traten ebenfalls signifikant häufiger (Chi-Quadrat-Test, p=0,0026) in der Testgruppe auf. Bei einem Vergleich der Gruppen in den Frakturtypen A.2.2, A.2.3, C.1.2 und C.2.2. ließ sich kein klinisch signifikanter Unterschied in der Fehlstellung in den Primärröntgen, in der Reponierbarkeit und im Halten des Repositionsergebnisses feststellen.
Schlußfolgerungen
Abgesehen von altersbedingten Unterschieden in der Häufigkeit von A.3-Frakturen und dorsalen Segmentinstabilitäten, konnten in diesem Design der Studie keine weiteren klinisch bedeutsamen Unterschiede nachgewiesen werden.
Summary
Background
It was the purpose of this study to describe age specific differences in the pathoanatomic roentgenographic appearance and in the AO classification system of conservatively treated distal radial fractures.
Methods
All 334 patients treated conservatively in 1988 at the Unfallkrankenhaus Linz. Austria, were included in an observational study. One testgroup of patients being 60 years old and older was compared with a controlgroup being 59 years old and younger. Data were collected retrolectively.
Results
There were 337 distal radial fractures in 334 patients. The median in the distribution of age was 59. A.3 fractures in the AO classification were significantly (Chi-square test, p=0.01) more frequent in the test group. Dorsal intercalated segment instabilities were significantly (Chi-square test, p=0.0026) more frequent in the test group. Patients having fractures of type A.2.2, A.2.3, C.1.2 and C.2.2 demonstrated no significant difference between test- and controlgroups (appropriate t-tests) in the roentgenographic appearance of the initial displacement of the fractured fragments, in the roentgenographic appearance of the realignment of these fragments and in the ability to hold the result until to the end of treatment.
Conclusions
Beside differences in the frequency of dorsal intercalated segment instabilities and in the frequency of A.3 type fractures no clinically significant differences have been detected with this study design.
