Zusammenfassung
Bei über Jahre progressiv verlaufender rheumatoider Arthritis finden sich häufig eine atlantoaxiale Instabilität mit Subluxation des Atlas sowie eine Arthritis der Temperomandibular- und Krikoarytaenoidalgelenke. Die mögliche Kompression des Rückenmarks aufgrund der atlanto-axialen Subluxation sowie die zu erwartenden Intubationsschwierigkeiten stellen für den Patienten ein besonders hohes anästhesiologisches Risiko dar. Die fiberoptische Intubation unter Spontanatmung und einer intravenösen Sedierung mit Propofol wurde im vorgestellten Fall erfolgreich angewandt.
Summary
Long-lasting states of rheumatoid arthritis may be associated with atlantoaxial subluxation, arthritis of the temporomandibular joints and cricoarytenoiditis. Possible compromise in spinal cord integrity due to atlanto-odontoid separation and difficulties in endotracheal intubation place these patients at a high anesthetic risk. Fiberoptic-guided intubation with the patient breathing spontaneously under intravenous anesthesia with propofol was carried out successfully in a patient with already compromized spinal function.
