Zusammenfassung
Die Problematik der Verhütung perioperativer Komplikationen wird durch 2 Aspekte geprägt: 1. zunehmende Tendenz zur ambulanten Chirurgie, auch bei Abdominaleingriffen, was organisatorisches Umdenken erfordert, 2 wachsende Zahlen alter und sehr alter Patienten, wobei auch die Operationsindikationen zunehmend liberalisiert werden.
Neben allgemeinen Altersschäden stehen Hypertonie, (chronische) kongestive Herzinsuffizienz und Myokardischämie, im Vordergrund therapeutischer Überlegungen. Vorrang vor jeder spezifischen Therapie haben ein sanierungsbedürftiger Elektrolytstatus (Na, Cl, Mg, speziell K) und Hypovolämiebehandlung gerade bei “trocken” gehaltenen Fällen—das Risiko einer feuchten Lunge wird meist weit überschätzt! Die bisherige Therapie mit Betablockern, Ca-Antagonisten, ACE-Hemmern usw. ist routinemäßig fortzusetzen; neuere Inotropiedrogen (Dopexamin, Amrinon usw.) harren noch eingehender Erprobung.
Die “stumme” Ischämie ist gegenwärtig das perioperative Aktualproblem. Akute Attacken bei leerer Anamnese werden häufig durch Tachykardien und Blutdruckanstieg ausgelöst—etwa während Intubation. Antihypertensiva (Ca-Antagonisten), eventuell in Kombination mit Betablockern sind hier angezeigt. Neuere nichtinvasive Überwachungsverfahren sowie Indiaktionen für eine passagere Schrifttmachersonde werden kurz abgehandelt. Trotz vieler Fortschritte bleibt in kritischen Fällen die klinische Sorgfalt des Anästhesisten der wesentliche risikomindernde Faktor.
Summary
Two main problems in the prevention of perioperative complications have evolved in recent years: (1) a growing tendency toward “out-patient” surgery, even for major abdominal operations; this will require a change with respect to preoperative patient evaluation etc. (2) an increasing number of old and very old patients, the indications for operation being nevertheless more and more liberalized.
In addition to general deficiencies occurring in old age special attention should be paid to hypertension, (chronic) congestive heart failure and myocardial ischaemia when reflecting upon therapeutic measures. Before applying any specific therapy electrolyte deficits have to be corrected (Na, Cl, Mg, K in particular) the same being true for treatment of hypovolaemia especially in “dry” cases—the potential risks of a “wet” lung are very often overrated!
Routine drug therapy with beta blockers, calcium antagonists, ACE inhibitors, etc. applied so far has to be continued; more recently developed inotropic drugs (dopexamine, amrinone, etc.) are still to be tested in detail.
At present “silent” ischaemia is to be consideredthe perioperative problem. In cases devoid of any anamnestic hints acute attacks frequently may be caused by tachycardia and/or acutely rising blood pressure, e.g., during intubation. Here the use of antihypertensive agents (calcium antagonists) possibly combined with beta blockers is indicated. More recent non-invasive monitoring methods as well as indications for transitory positioning of a pacemaker probe are briefly dealt with. In spite of all “technical” progress in these fields in critical cases constant alertness and clinical conscientiousness warranted by the anaesthetist still are to be regarded asthe main risk-reducing determinants.
