Zusammenfassung
Bereits die Frühphase eines mechanischen Ileus führt zu komplexen Veränderungen im Neuroendokrinium des Darms. Es wurde experimentell ein Obstruktionsileus erzeugt. Die Ergebnisse:

Der intragastrale Druck steigt bei hohem Ileus kontinuierlich an, der Intraluminäre bleibt unverändert, obwohl der Darm distendiert wird.

Die Parameter der Blutgerinnung, der pulmonalen Funktion und der Nierenfunktion sind in den ersten 8 Stunden poststenotisch unverändert.

Endotoxin ist nach 4 Stunden im Blut nachweisbar.

Morphologische Veränderungen sind wenig ausgeprägt, die Darmenzyme, vor allem CPK und deren Isoenzyme, zeigen geringe Verschiebungen, die auf Gewebsuntergang schlleßen lassen.

Parallel zum Endotoxinanstieg wird Beta-Endorphin ausgeschüttet.

Katecholamine steigen massiv an.

Thyroxin und Trijodthyronin sinken auf die Hälfte des Basiswertes ab. Mit dem Opiat-Rezeptor-Antagonisten Thyrotropin-releasing hormone (TRH) (1 mg/kg Körpergewicht) kann gegenüber der unbehandeiten lleusgruppe folgendes festgestellt werden:

Beta-Endorphinwerte und Katecholamine sind signifikant niederer. Endotoxin ist erst nach 6 Studen nachweisbar. T3/T4-Spiegel sind konstant zur Ausgangslage.
Unabhängig von chirurgischer Sanierung ist ein therapeutischer Effekt durch TRH durch verminderte Werte vasoaktiver Substanzen und einer Hypophysenstimulation gegeben.
Summary
Endocrine changes in early intestinal occlusion were investigated experimentally. Compared to controls, the following differences were evaluated:
u li]Intragastral pressure rises in high obstruction, whereas intraluminal pressure stays unchanged despite massive gut distension. li]Endotoxin appears already after 4 hours of occlusion in blood. li]Creatin-phosphokinase and iso-enzyms show little morphological changes of gut tissue. li]Beta-endorphin appears parallely to endotoxin and rises constantly as well as plasma catecholamins. li]Thyroxin and triode-thyronin decrease constantly.
Even at a very early stage of occlusion vasoactive substances arise, very similar to „early shock state”.
Thyrotropin-releasing hormone (TRH) blocks endorphin activity. In the therapy group 1 mg/kg bodyweight TRH was injected, leading to the following changes compared to the ileus group: li]Beta-Endorphin and catecholamin levels are significantly lower, endotoxin appears at 6 hours, T3/T4-levels stay constant. The beneficial effect of TRH in early obstructive ileus is documented by the lower levels of vasoactive substances and pituitary stimulation, independent of the need of surgical intervention.
