Zusammenfassung
Die Krankengeschichte eines 42jährigen Patienten mit einem durch Arterieniigatur erfolgreich behandelten iatrogenen Aneurysma der A. hepatica dextra wird vorgestellt. Iatrogene Hepatikaaneurysmen treten nach Literaturangaben mehrheitlich nach Gallenwegssystemoperationen in der A. hepatica dextra auf (72%) und führen bei 87,5% zur Hämobilie, die durch die Quinke'sche Trias charakterisiert ist. Als Therapie wird wegen der meist unübersichtlichen postoperativen Verhältnisse die A. hepatica-Ligatur empfohlen, obwohl auch dieses Verfahren häufig (45%) einen letalen Ausgang nimmt.
Summary
The history of a 42-year old patient with a right hepatic artery aneurysm is presented, which was treated successfully by ligature of the right hepatic artery. According to literature data iatrogenous aneurysms of the hepatic artery mainly occure following biliary system operations in the right hepatic artery branch (72%) causing hemobilia in 87,5% characterised by the Quinke symptom complex. Because of the difficult postoperative anatomic situs ligature of the hepatic artery is recommended for treatment in literature, though operative mortality following this procedure is high, too.
