Zusammenfassung

In dieser Arbeit werden die Nebeneffekte der i.v. Gammaglobulintherapie analysiert: Bei der Verabreichung von 73 Dosen à 10 g von ungespaltenem Globulin an Patienten mit humoralen Immundefekten sahen wir bei 9 Fällen schwere, teils lebensbedrohliche, aber voll reversible Komplikationen. Allen diesen Patienten gemeinsam war die initiale Antigenämie, die über die Zone der Äquivalenz in den Zustand des Antikörperüberschusses gebracht werden mußte.

Immunkomplexbestimmungen ergaben ein meßbares Ansteigen derselben bei 4 von 9 Fällen, bei denen wir auch gleichzeitig ein Absinken von C 4 feststellten. Daraufhin haben wir unser Therapieprotokoll geändert und Gammaglobuline 5 S vor 7 S appliziert. Bei den folgenden 148 Therapien sahen wir nie mehr vergleichbare Nebeneffekte.

Summary

This is a contribution on the side effects of i.v. gammaglobulin in patients with immunodeficiency. 73 therapies were carried out, 9 patients suffered from partially life threatening complications. All side effects were fully reversible and occured during the transition from antigenaemia to antibody-excess. Immune-complexes could be quantitated and C 4 was diminished. In view of these results we have modified our protocol giving 5 S gammaglobulin prior to 7 S. No further complication was seen in the following 148 therapies.

Hence we can conclude, that at least some complications of the i.v. gamma-globuline-therapy is due to the formation of immunecomplexes and can be prevented by the application of 5 S prior to 7 S.
