Über Ursache und Verhütung hämodynamischer Komplikationen bei der Arterialisation des intrahepatischen Pfortadersystems nach portokavalen Anastomosen

Zusammenfassung

Es wird auf eine bisher unbekannte Komplikation der Arterialisation der intrahepatischen Pfortader nach portokavalen Anastomosen hingewiesen, den Abfluß des zugeleiteten arteriellen Blutstromes aus dem linken intrahepatalen Pfortaderhauptast hepatofugal über spontan offene oder sekundär eröffnete Kollateralen zu Fundus- und Ösophagusvarizen. Diese Komplikation kann nur erkannt werden, wenn postoperativ routinemäßig eine Kontrollangiographie ausgeführt wird. Der einfachste Korrektureingriff ist die Ligatur des Veneninterponates mit Unterbrechung des arteriellen Zustromes der Leber. Die segmentale Arterialisation der rechten Leberhälfte mit Ligatur des linken intrahepatalen Pfortader-Hauptastes oder die Durchtrennung sämtlicher, vom linken Leberlappen abgehender venöser Kollateralen sind wesentlich aufwendigere Verfahren, die weniger als Korrektur-, sondern vielmehr als Präventiveingriffe im Rahmen der Erstoperation zur Diskussion stehen.

Summary

This is a report of a hitherto unknown complication of portocaval shunt with portal arterialization. Arterial run off occurred in a hepatofugal direction via pre-existing or secondary recanalized collaterals from the left intrahepatic portal main branch to esophageal and fundus varices.

This complication can be diagnosed only by routine postoperative angiography. To correct this situation the vein graft may be ligated by which procedure the arterial inflow is interrupted. To prevent this complication one could also arterialize the right half of the liver with ligation of the left intrahepatic portal main branch. Another possibility is to ligate all venous collaterals originating from the left hepatic lobe.
