Zusammenfassung
Ösophaguskarzinom
Das Frühergebnis des Berichtsabschnittes 1966 bis 1969 (104 Patienten) unterscheidet sich von dem des Berichtsabschnittes 1970 bis 1974 (106 Patienten) durch den Anstieg der Resektionsrate von 26 auf 35% und eine Senkung der Operationsletalität von 52 auf 16%. Diese Verbesserung wird vor allem auf die Einführung der Mehrstadienoperation (subtotale Ösophagektomie und retrosternale Koloninterposition) zurückgeführt.
Kardiakarzinom
Taktisches Konzept auf Grund von 288 abdominellen Gastrektomien, 34 abdominellen Kardiaresektionen, 13 abdominolinksthorakalen, 13 linksthorakotransdiaphragmalen Kardiaresektionen und 15 abdominothorakalen Gastrektomien aus 16 Jahren:

Primär abdominelles Vorgehen.

Nur beim kleinen kardianahen Karzinom abdominelle Kardiaresektion, bei größerer Erfassung des Magens Gastrektomie.

Beim alten Patienten ist rein abdominelles Erzwingen der Operation auch unter Verzicht auf größeren oralen Tumorabstand gestattet und empfehlenswert.
Summary
Carcinoma of the esophagus
The preliminary results of the report covering the period between 1966–1969 (104 patients with diagnosed carcinomas admitted to the hospital) differentiate from those of the 1970–1974 period report (106 hospital admitted carcinoma patients) owing to an increase in the resection rate from 26% to 35% and a decrease in the operative mortality rate from 52 to 16%. This improvement may above all be attributed to the introduction of a multiple staged operational method (subtotal esophagoectomy and retrosternal interposition of the colon) that now is used at the clinic as the standard procedure for operations involving the upper two thirds of the thoracic section of the esophagus.
Carcinoma of the cardia
The evaluation of the results of 288 abdominal gastrectomies, 34 abdominal cardia resections, 13 left abdominothoracic, 13 left thoracic transdiaphragmatic cardiaresections, and 15 abdominothoracic gastrectomies over 16 years confirm our tactical concept:

Primary transabdominal approach.

Cardia resection only in cases of small carcinomas; in cases of more extended tumors of the stomach gastrectomy must be performed!

In the case of elderly patients it is permitted or rather advisable to strictly restrain the operation to the abdominal approach even at the risk of reducing the interval between tumor and the oral resection site.
