Rekonstruktive Eingriffe am Bronchialsystem

Zusammenfassung

An Hand von 3 Fällen werden einige Probleme, die im Zusammenhang mit rekonstruktiven Eingriffen am Bronchialsystem auftreten, besprochen. Es wird darauf hingewiesen, daß die Bedingungen, die nach Korrektur einer Kontinuitätstrennung eines Bronchus zur Wiederherstellung der Gaswechselfunktion der betroffenen Lungenabschnitte nötig sind, bisher kaum bekannt sind. Auf das operative Vorgehen im Fall einer Totalresktion der Trachealbifurkation wird eingegangen und eine bisher nicht beobachtete lebensbedrohliche Komplikation (postoperative Bronchomalazie) und deren palliative Beherrschung beschrieben.

Summary

A review of some problems concerning reconstructive surgery of the bronchial system is given on the basis of 3 cases. It is stressed, that the conditions for restoration of gas exchange in the concerned parts of the lung after repair of a disconnected bronchus are at present mostly unknown. The operative management in a case of resection of the tracheal bifurcation is described and an until now unknown dangerous complication (postoperative brochial malacia) and its palliative treatment is discussed.
