Zusammenfassung
Frakturhämatome fanden bisher wenig allgemeine Beachtung, obwohl ihnen im posttraumatischen Geschehen eine Bedeutung zukommen kann.
Hämatome um geschlossene Frakturen verschiedener Lokalisation können oft beachtliche Ausmaße erreichen und schon dadurch hämodynamische Allgemeinreaktionen hervorrufen.
Nach experimentell erzeugten Oberschenkelfrakturen und anschließender Blutentnahme konnten wir eine bereits bestehende, posttraumatische Hyperlipämie noch wesentlich verstärken. Aus dieser am Versuchsmodell gemachten Beobachtung ergibt sich die klinisch bestätigte, therapeutische Konsequenz einer ausreichenden Volumensubstitution auch bei geschlossenen Knochenfrakturen.
Als Ursache für die Ungerinnbarkeit des Hämatomblutes um Frakturen verschiedener Lokalisation konnten wir einen gesteigerten Gehalt an fibrinauflösenden, proteolytischen Aktivitäten nachweisen, die noch 3 Tage nach dem Frakturereignis vorhanden waren.
Eine Hemmung dieser Aktivitäten konnte, im Gegensatz zu Serum aus dem Gefäßsystem derselben Patienten, durch natürliche Proteinasen-Inhibitoren erreicht werden.
Die Prüfung der lokalen Inhibitorapplikation im Anschluß an experimentell erzeugte Frakturen an Ratten ergab in bezug auf die Knochenbruchheilung eine signifikant festere Konsolidierung der Knochenfragmente, als bei den mit gleichen Volumina Kochsalzlösung behandelten Kontrolltieren.
Durch diese Untersuchungen und Ergebnisse wird auf die fibrinauflösende Eigenschaft des Frakturhämatomes hingewiesen und es werden Möglichkeiten der Hemmung dieser Aktivitäten aufgezeigt, da ein suffizientes Fibringerüst der Beginn aller reparativen Vorgänge ist.
Summary
In hematomata around fractures there is a rise in the plasminogen activators accompanied by a reduction of the plasminogen and a significant increase of the fibrinolytic and proteolytic enzymes to be observed. All these changes occur in comparison with the venous blood of the same patient.
Because in a hematoma the fibrin splits are present in increased quantities, the absence of fibrinogen must be due to the completed process of digestion: the inhibitor potential against plasmin is exhausted.
Contrary to the results obtained from the serum of the same patient's vascular system, we were able to bring about an inhibition of this fibrinolytic and proteolytic enzyme activity in fracture hematomata by means of natural proteinase inhibitors.
In rats we induced fractures experimentally and examined the healing process after local inhibitor application. We found a significantly better consolidation of the bone fragments in the test animals as compared to controls that had been administered the identical volume but of saline solution.
Experiments and the results derived thereform seem to confirm the fibrinolytic property of the fracture weisen waren, wurden die Versuche an Ratten ausgeführt.
