Zusammenfassung
Nach kurzer Beschreibung einer chirurgischen Funktionsprüfung des Sphinkter Oddi und einem kurzen Literaturüberblick über Physiologie und Pathologie dieser Region werden an Hand eigener Ergebnisse die Formen des Galleaustrittes an der Papilla Vateri erörtert.
Mittels Elektromanometrie mit konstanter Durchflußmenge läßt sich eine Bewegungsform des Sphinkterapparates abgrenzen, die als große Sphinktermotilität bezeichnet wird und möglicherweise als physiologischer Akt bei sehr kräftiger Galleentleerung auftritt. Wichtiger ist jedoch die Tatsache, daß diese Bewegungsform beim Patienten während der Operation nur ausnahmsweise und vor allem dann beobachtet wird, wenn sich eine Papillenveränderung vorfindet, die im Sinne vonBergh als “irritable sphincter” bezeichnet wird. Dazu rechnen wir Papillitis, Papillenstenose, Konkrement; in einer zweiten Gruppe operationsbedingte oder untersuchungsbedingte Irritation der Papillenregion.
Die nervöse Regulation der Sphinkterbewegungen betrifft den Muskeltonus, nicht aber den Bewegungsrhythmus. Dieser wird wahrscheinlich von der glatten Muskulatur selbst ausgelöst.
Mit Einführung des Begriffes einer Sphinkterirritation wird das Verständnis der zu beobachtenden, pathologischen Sphinkterbewegungen (große Sphinktermotilität, Spasmus) erleichtert. Wir verstehen darunter Störungen, die von der glatten Muskulatur selbst ausgehen.
Summary
After a short description of a surgical function test of the sphincter Oddi and a brief bibliographical review on the physiology and pathology of this region the forms of biliary passage at the papilla of Vater are discussed in the light of results found by the author.
With electromanometry under constant flow a pattern of motility of the sphincter may be defined which is called great sphincter motility and possibly occurs as physiological reaction with excessive gall flow. Important, however, is the fact, that this form of motility is seen with the patients during the operation only exceptionally, above all when there is a lesion of the papilla, meaning “irritable sphincter” according toBergh. By this term we understand papillitis, stenosis of the papilla, concretion; in a second group irritation of the papilla region due to operation and/or examination.
The nervous regulation of sphincter activity affects the tone of the muscle but not the rhythm of motility. The latter is probably caused by the smooth muscle tissue itself.
Introducing the term of “irritable sphincter” helps to understand the observed patholgic movements of the sphincter (great sphincter motility, spasm). We define it as disturbances starting from the involuntary muscles.
