Eine seltene Beobachtung bei Verrenkungen im Daumengrundgelenk

Zusammenfassung

• 1. Eine typische Luxation im Daumengrundgelenk wird beschrieben, die nach Reposition eine federnde Fixation vortäuschte.

• 2. Röntgenologisch wurde eine fast ebene Gelenksfläche im Daumengrundgelenk des Capitulum Metacarpale I und Grundphalanx gefunden.

• 3. Diese Form des Gelenkes wurde von der Mutter vererbt, die eine gleichartige Gelenksfläche im Daumengrundgelenk zeigte und auch eine fast völlige Bewegungsunfähigkeit des Gelenkes zeigte.

• 4. Bei einer Kontrolluntersuchung an 50 verschieden alten und verschiedengeschlechtlichen Patienten wurden die Gelenksflächen röntgenologisch geprüft und die Beweglichkeit gemessen.

• 5. Ein Zusammenhang des Bewegungsumfanges mit dem anatomischen Bau der Gelenksoberfläche konnte eindeutig bewiesen werden.

• 6. Die bisherige Ansicht, daß die Beweglichkeit nur von der Straffheit und Beschaffenheit des Band-und Kapselapparates abhängig ist, wird bezweifelt.

Summary

• 1. In a typical metacarpophalangeal luxation of a thumb after reduction the symptom of “elastic fixation” was noticed—the mobility of the joint was almost nil.

• 2. In x-ray pictures the articular surfaces of the phalangeal base and of the capitulum of the metacarpal bone appeared to be almost flat.

• 3. The same appearance of the articular surfaces was found in x-ray pictures of the patients mother. Also here the mobility of the joint was almost abolished.

• 4. In 50 patients of either sex and of different age groups x-rays of the metacarpo-phalangeal joint of the thumb were taken and the mobility of this joint was examined.

• 5. The mobility rate was dependent from the shape of the articular surfaces. Flat surfaces permitted a very restricted mobility only, whereas in round, ball-shaped surfaces always an improved mobility was observed.

• 6. The elasticity of the joint ligaments and of the capsula seemed to be of little influence on the rate of mobility.
