Zusammenfassung
Nach Abhandlung der stumpfen und penetrierenden Lungenverletzungen wird auf die dringliche Beseitigung des Hämopneumothorax eingegangen. Wenn die völlige Lungenexpansion nicht durch Aspiration und Punktion gelingt, soll sofort eine geschlossene Saugdrainage angelegt werden. Die Thorakotomie ist bei nicht sistierenden legt werden. Die Thorakotomie ist bei nicht sistierenden Blutungen, Gasverlusten von mehr als 40% des Atemminutenvolumens und intrapulmonalen Fremdkörpern ab 1,5 cm Durchmesser angezeigt.
Sorge um freie Atemwege, Schockbekämpfung und O2-Therapie sind vordringliche Maßnahmen. Hypoventilation und Störung des Gasaustausches (pO2 art. <80 mm Hg; pCO2>60 mm Hg) indizieren die künstliche Beatmung mit Respiratoren, die nur bei Explosionsverletzungen und Compressio thoracis kontraindiziert ist.
Bei Zweihöhlenverletzungen ist die Laparotomie vorrangig, da die Verhältnisse im Thorax nach Anlegung einer geschlossenen Saugdrainage immer besser zu überblicken sind.
Summary
After discussing the management of blunt and penetrated injuries of the lung, the importance of controlling the haemopneumothorax is emphasized. When full expansion of the lungs by means of aspiration or punction is not possible, a closed suction-drainage is to be applied immediately. Thoracotomy is always indicated when the bleeding is not controlled, when there is a loss of 40 p. c. of respiratory volume per minute, and when there is an intra-pulmonary foreign body of dimensions larger than 1.5 cm present.
Restoration of a free respiratory passage, treatment of shock and O2-therapy are important. Hypoventilation and disturbances of the gas exchange (pO2 art.<80 mm Hg; pCO2 art.>60 mm Hg) require immediate artificial respiration—excepting cases of explosive or compressive injuries.
In cases of combined thoracic and abdominal injuries, the laparotomy should be performed first. Application of a suction-drainage in the thorax would be sufficient for the moment—at least until a decision as to the necessity of a thorax surgery can be taken.
