Zusammenfassung
Bis auf 3 Kinder waren unsere Patienten alles Jugendliche oder Erwachsene, 6 Mädchen, 12 Burschen. Bei einem schwer herzgeschädigten Patienten kam es wenige Stunden nach der Operation zum akuten Herzversagen und zum Tod. Bei einem Mädchen durch Hautrand-nekrose p.s.-Heilung und Fistelbildung, die eine Sekundäroperation erzwang. Der Dermislappen blieb aber erhalten und die Thoraxstabilität somit gewährleistet. Bei allen anderen kam es weder unmittelbar postoperativ noch später zu einer Komplikation.
Der postoperative Spitalsaufenthalt betrug bei den Dermislappen durchschnittlich 18 Tage, bei den gekreuzten Drähten 15 Tage, war also auffällig kurz. Die Patienten konnten ab dem zweiten postoperativen Tag das Bett verlassen. Zu irgendwelchen Atmungs- oder Expektorationsstörungen kam es nicht.
Bis jetzt haben wir bei unseren Techniken noch kein Rezidiv erlebt. Sowohl die funktionellen wie die kosmetischen Ergebnisse sind durchwegs gut. Wir halten daher die beschriebene Technik für eine echte Erweiterung der Möglichkeiten zur operativen Korrektur der Trichterbrust.
Summary
Various operative techniques, vastly different from each other, have been described to cure “funnel-chestdeformity”. After diseussing the literature, personal experiences gained from 18 adult patients are described. In order to minimize complications and insure sufficient blood-supply, both internnal mammary arteries must be preserved in all cases, 2 main groups of the deformity have to be distinguished: 1. Deep local depressions of the sternum where not only the ribs must be divided but the breastbone itself must be cut into comparatively many and small pieces. Here, a dermisgraft taken from the thigh and tightly sutured under the fragments gives the required stability and makes the thorax water- and airtight (14 cases). 2. Extensive, flat depressions where a repositioning is possible, leaving big bony fragments together. Here one or two pairs of Kirschner wire are drilled through ribs and sternum, crossing in the midline. They should be bent into some overcorrection and be left in place for about 6 months. Both methods give uniformally satisfying results if proper care in selecting the patients is taken.
