Zur Autoreplantation des Daumens

Zusammenfassung

An Hand von 3 Fällen haben wir unsere Technik der Autoreplantation des Daumens nach traumatischer Amputation vorgestellt, die sich von der gängigen Literatur in der Erhaltung eines größtmöglichen Weichteilmantels nach Entfernung der dünnen Hautdecke und in einer zeitlich fast doppelt so langen Blutversorgung durch den Lappenstiel bis zur endgültigen Durchtrennung unterscheidet. Die normalen anatomischen Verhältnisse sind die beste Voraussetzung für eine befriedigende Schutzsensibilität.

Summary

Our technique of autoreplantation after traumatic amputation of the thumb is discussed and illustrated by three cases. Contrary to description in the literature, only the skin from the amputated part was very carefully taken off. The remaining soft tissue is not touched so that the anatomical structures especially of the finger-tip are preserved. At the same time a suture of the collateral nerve without microscope was tried. A one to three years follow-up shows no signs of secondary bone-resorption. In one case, where the matrix of the fingernail was preserved, a nearly perfect fingernail reappeared after six months.
