Gli AA. hanno confrontato, in soggetti con sospette turbe della glicoregolazione, il comportamento del test di tolleranza al glucosio p.o. eseguito al mattino e nel pomeriggio. Il test pomeridiano è risultato significativamente più valido agli effetti della diagnosi di diabete subclinico e, quindi, preferibile ad altre metodiche. Esso rappresenta infatti un mezzo di indagine del tutto fisiologico, inserendosi nella fase circadiana dell’esaurimento fisiologico della dismissione insulinica. In un gruppo di controllo costituito da 8 diabetici di media gravità, le curve relative ai tests antimeridiano e pomeridiano sono apparse sovrapponibili.
Résumé
Les Auteurs ont comparé le comportement du test de tolérance au glucose oral effectué le matin et dans l’après-midi chez des sujets dont on supposait des troubles de la glycorégulation. Le test de l’après-midi s’est avéré sensiblement plus valable pour le diagnostic du diabète subclinique et, par conséquent, préférable par rapport à d’autres méthodes. En effet, il représente un moyen d’enquête tout à fait physiologique, car il s’insère dans la phase circadienne de l’exhaustion physiologique pendant la libération de l’insuline. Chez un groupe de contrôle comprenant 8 diabétiques de gravité moyenne, les courbes relatives aux tests du matin et de l’après-midi pouvaient être superposées.
Resumen
Los autores han comparado la evolución del test de tolerancia a la glucosa p.o. a que fueron sometidos por la mañana y por la tarde, individuos en los que se sospechabe la presencia de turbaciones de la glucorregulación. A efectos del diagnóstico de la diabetes subclínica, el test vespertino demostró ser decididamente preferible y, por tanto, constituir un medio de investigación totalmente fisiológico que se relaciona con la fase circadiana del agotamiento de la capacidad insulinogénica. En un grupo de control constituido por ocho diabeticos de relativa gravedad, las curvas correspondientes a los test matutino y vespertino resultaron superponibles.
Zusammenfassung
Die Verfasser verglichen bei Probanden mit Verdacht auf Störung der Glykoregulation das Verhalten der oralen Zuckerbelastungsprobe am Vormittag und am Nachmittag. Die Nachmittagsprobe erwies sich als entschieden zuverlässiger für die Diagnose des subklinischen Diabetes und ist daher anderen Methoden vorzuziehen. Sie stellt eine physiologische Methode dar, da sie zeitlich der normalen Erschöpfungsphase der Insulinfreisetzung entspricht. Bei einer Kontrollgruppe von 8 Fällen von mittelschwerem Diabetes fielen die Vormittags- und die Nachmittagskurve zusammen.
Summary
The authors compared the behaviour of morning and afternoon oral glucose tolerance tests in patients in whom disorders of carbohydrate metabolism were suspected. The afternoon test resulted significantly more useful for the diagnosis of subclinical diabetes and, therefore, better than other methods. It is, in fact, a physiological method since it is timed so as to coincide with the normal stage of exhaustion of insulinogenesis. In a control group of 8 subjects with diabetes of medium severity, the morning and afternoon curves were identical.
