Zusammenfassung

Stumpfe Herzverletzungen treten im Gefolge von Thoraxtraumen weit häufiger auf als allgemein angenommen. Wegen bestehender Begleitverletzungen ist ihre Diagnostik oft sehr erschwert. Während leichtere Traumen auch ohne Therapie völlig ausheilen, können schwerere Formen wegen eines Hämoperikards oder wegen akuter Herzinsuffizienz (z. B. bei traumatischen Klappenfehlern) die sofortige operative Therapie notwendig machen. Unter Umständen ist der Einsatz der Herz-Lungen-Maschine angezeigt.

Penetrierende Herzverletzungen machen im Hinblick auf die Diagnostik in der Regel wenig Schwierigkeiten. Wegen der hohen Frühsterblichkeit ist hier vor allem die rechtzeitige klinische Behandlung vordringlich.

Iatrogene Herzverletzungen nehmen im Rahmen der Technisierung der Medizin ständig zu. In der täglichen Praxis sind hier besonders die Komplikationen nach Anwendung des Kava-Katheters von Bedeutung.

Abstract

Closed injuries to the heart after thoracic trauma are much more common than it is generally recognized. It is sometimes difficult to diagnose these cases, as the patients have suffered multiple injuries. Minor injuries might recover without any treatment; in severe cases immediate surgical intervention is necessary because of hemopericard or acute cardia failure (for instance after traumatic valvular lesions). Sometimes extra-corporal circulation is needed.

Perforating injuries to the heart are easily recognized. Because of a high early mortality treatment in hospital should be attempted without delay.

Iatrogenic injuries to the heart are seen more and more. They are due to the increasing use of technical procedures in medicine. In the daily hospital routine especially complications after catheterization of the vena cava are common.
