Die Plattenosteosynthese mehrfacher Rippenbrüche zur Stabilisierung der Thoraxwand

Zusammenfassung

Bei sechs Patienten mit Serienrippenbrüchen und paradoxer Atmung wurde im Rahmen der notwendigen Thorakotomie auch eine Verplattung von maximal sechs Rippen mit Drittelrohrplatten und Kleinfragment-Kortikalisschrauben vorgenommen. Zwei Patienten sind an anderen Ursachen als der Thoraxverletzung verstorben. Eine Patientin wurde 36 Stunden lang beatmet, drei Patienten zeigten nur mit nasaler Sauerstoffzufuhr ohne Respiratorbehandlung eine Erholung. Bei diesen vier Überlebenden sind keine wesentlichen Komplikationen eingetreten. Es wird damit neuerlich bestätigt, daß die direkte operative Stabilisierung der Rippenbrüche beim unstabilen Thorax einen positiven Effekt hat.

Abstract

6 patients with severe chest injuries and paradoxical respiration were operated and the rib fractures stabilized by means of small plates and small fragment screws. 2 patients died due to other causes, only 1 patient had to be treated with a volume cycled respirator for 36 hours. The other 3 patients needed only nasal oxygen insuffiation and made very good recoveries. There were no serious complications. We feel that in severe chest injuries more active surgical treatment is justified. Stabilizing operations reduce the amount of needed artificial respiration and can be strongly recommended.
